Objective: Detection of plasma copeptin concentration in patients with chronic heartfailure,analysis on the change trend of copeptin concentration in plasma of patients withchronic heart failure,to investigate correlation between copeptin and cardiac function inpatients with chronic heart failure;To provide a new method and way for evaluatingcardiac fuction in patients with chronic heart failure;At the same time to discuss detectingcopeptin and BNP of the clinical application value in patients with chronic heart failure.Methods: Collect83cases of patients with chronic heart failure, including35cases ofischemic cardiomyopathy,27patients with dilated cardiomyopathy, hypertensive heartdisease in21cases, establish30cases of health control group.According to the AmericaNew York Heart Association (NYHA) cardiac function grading criteria in patients withchronic heart failure were divided into four groups:NYHAⅠ group17cases,NYHAⅡg roup of22cases, NYHA Ⅲg roup of24cases, NYHA Ⅳgroup of20cases. RecordingBaseline information of the research object,By using enzyme linked immunosorbentassay (ELISA) were determined all selected objects copeptin content and BNPcontentin,using XMATRIXIE33color Doppler ultrasound diagnostic apparatus (PHILIPS)determine left ventricular end diastolic diameter (LVEDd) and left ventricular ejectionfraction (LVEF). Statistical analysis of data using the statistical package of SSPS20.0software.Results: Chronic heart failure group compared with control group, the increase of plasmacopeptin level, there was significant difference (p <0.001),each sub group of chronicheart failure group plasma copeptin levels compared with the control group, there wassignificant difference (p<0.05),chronic heart failure between each sub group also showedsignificant difference (p<0.05),and withing the NYHA cardiac functional grading ofelevated plasma copeptin level increased;Cardiac function in chronic heart failure groupplasma copeptin level and NYHA grade were positively correlated (r=0.876,p<0.001),was positively correlated with LVEDd (r=0.515,p<0.001), significantly negative correlation with LVEF (r=-0.711,p<0.001), and with the level of BNPwere highly positively correlated(r=0.774,p<0.001);The results of ROC curveanalysis:copeptin、BNP single and combined detection of Youden index at the maximumsensitivity, specificity and the area under the ROC curve (AUC) were:93.5%,86.3%,0.95(95%confidence interval:0.89~0.98);90.3%,82.4%,0.92(95%confidence interval:0.86~0.96);95.2%,84.3%,0.96(95%confidence interval:0.90~0.99);Through the areaunder the ROC curve Comparison shows: there was significant difference between BNPand BNP combined with copeptin detection (p<0.05).Conclusion: Elevated in chronic heart failure patients plasma copeptin levels, along withthe deterioration of cardiac function in patients with plasma copeptin levels continued torise, the NYHAII group, NYHAI group and control group, plasma copeptin levels werestatistically significant difference (p<0.05); Tips copeptin for chronic heart failurediagnosis and objective evaluation of severity of cardiac function has clinicalreference value, especially for the early diagnosis of heart failure have some clinicalvalue. Copeptin with BNP, echocardiographic parameters of heart failure are closelyrelated, Further tips the copeptin can be used for the diagnosis and objective evaluationof cardiac function; copeptin sensitivity of early heart failure and short-term changesof cardiac function and other aspects of heart failure were higher than echocardiographicparameters, ROC curve analysis showed that the sensitivity and specificity of copeptinwere higher than BNP, Therefore, in patients with chronic heart failure plasma copeptinlevels are a strong complement to shortcomings of echocardiography and the BNP inchronic heart failure clinical application process, meanwhile ROC curve showed copeptincombined BNP testing to improve the accuracy of clinical diagnosis of chronic heartfailure. So copeptin has important clinical value in patients with chronic heart failure,After expected to become the BNP, clinical work another new biochemical markers ofheart failure.
