| Purpose and significance:In this research, a comparative study between haemorrhoids ligation that was combinated the haemorrhoids mucosa ligation with haemorrhoids basal ligation and traditional haemorrhoids through ligation in the treatment of haemorrhoids at Ⅱ and Ⅲ stage in order to observe the clinical effect, discusses its treatment mechanism, provide theoretical basis for clinical application.methods:In this research,60 patients with haemorrhoids atⅡ and Ⅲ stage were randomly individed into 30 cases haemorrhoids ligation in treatment group (haemorrhoids mucosa ligation method joint haemorrhoids basal ligation method of haemorrhoids ligation operation) and 30 cases through ligation in control group (traditional through ligation). Overall clinical effects, anal inflation and bleeding integral distribution in the surgery day and in the postoperative 1,3,7 day, haemorrhoids shedding time, as well as clinical effect after 6 months follow-up, were observed in the two groups.Results:1.The comparison in the total effective rate in two groups:There are 29 cases cure and 1 case improve in the treatment group and there are 28 cases cure and 2 cases improve in the control group,the total effective rate in two groups is 100%.Total effective rate in two groups were no significant difference (P>0.05);2. The comparison of anal inflation in the postoperative in two groups:anal inflation in the surgery day and in the postoperative 1,3 day in two groups was significant difference (P<0.05), and wasn’t significant difference (P>0.05) in the seventh day after operation, which were suggested that:early postoperative anal bulge in the early stage after operation is more in treatment group than in control group;3. The comparison in the postoperative bleeding situation in two groups:There were no massive haemorrhage in the surgery day in two groups, bleeding points distribution in two groups were significant differences (P<0.05) after operation in 1,3,7 day by mann-whitney test, postoperative bleeding in the treatment group is much better than the one in the control group;4. The comparison of hemorrhoids shedding time in two groups:It is (6.93±1.413) days in the treatment group, and (11.5±1.503) days in the control group. There were significant difference (P<0.05) in two groups, which was significantly better in the treatment group than in the control group;5. The comparison in effect of 6 months follow-up in two groups:There were no recurrence and other complications in the treatment group and there was lcase haemorrhoids incomplete atrophy and haemorrhoids were no hernia and bloody in the control group.There were the same effect in two groups and no significant statistical significance by mann-whitney test (P=0.317>0.05).There were no anal stricture and anal dysfunction in 6 months follow-up in two groups, which was no statistical significance. Two groups of patients with postoperative follow-up of 6 months no anal stricture occurred and anus control dysfunction, no statistical significance.Conclusion:There are the same effect in the haemorrhoids ligation and through ligation in the treatment of haemorrhoids at Ⅱ and Ⅲ stages, but the haemorrhoids ligation has advantages in less complications, simple operation, shorter operation time and haemorrhoids shedding time and low recurrence rate, operation simple easy to grip. |
