At present, China’s public hospitals are divided into general hospitals, Chinesemedicine hospitals, specialist hospitals and primary health care institutions. Theformer three main provider of comprehensive and integrated medical services. And asa primary health care institutions, community health centers is undoubtedly the mostimportant part of its. There is no doubt that the grassroots community health servicecenter determines its key position in the basic medical service system as thefoundation of the city public health, the functioning of the public health and the basicways of achievement. In the new round of medical and health system, how to developcommunity health services to alleviate the problem that medical treatment is difficultand expensive? This is the breakthrough of health system reform.Community Health Center is organized and managed by government. However,there are a variety of administrative problems, especially in the reform of familydoctor service. The existing organization structure has not conducive to the currentcommunity functions, needs to be optimized.Family doctor system is based on a GP team, in order to provide comprehensivehealth management as the goal. In the family unit, family doctor system providescontinuous, safe, effective, appropriate, comprehensive health care services and healthmanagement model for every member of the family. In a district Shanghai, forexample, to cope with the family doctor to carry out the work, town governmentcreates the community management division. This division in charge of themanagement of the family doctor team, which promote the smooth development of community health services. However, community management division is essentiallya temporary task team and lack of organizational support. All these are not conducivefor effective and efficient management of a team of doctors and family doctors work.On the other hand, the family doctor’s work makes six parts in one function ofcommunity health, namely, prevention, health care, medical care, rehabilitation,healtheducation and family planning technical guidance and service function is moreprominent. From the past to the outpatient department transition team service, socialinteraction and the team’s collaboration is getting enhancement, and rises the newrequirement to breakout the organization’s boundary. At the same time, repositioningthe community first diagnosis system and two-way referral system betweencommunity health function, the community health service centers and medicalinstitutions at all levels will have a new channel for contact with the new operationmechanism.Therefore, if family doctor’s work want to develop smoothly, organizationalstructure reform is indispensable. This paper will start from the perspective of thetheory of organization architecture, from the international community healthdevelopment experience, combined with the community functions, explore theoptimal combination of management efficiency and the degree of division of labor,the new management concept of performance management, resource sharing,socialparticipation mechanism into the framework structure reform to go,to form the designorganization system to promote community health service center.
