Objective To explore the elderly pulmonary tuberculosis epidemic characteristic and trend from 2005 to 2013 in Dongchuan district through analyzing tuberculosis distribution, the diagnosis delay and treatment outcome among the elderly tuberculosis patients; Through comparing the changes about tuberculosis core knowledge in the elderly, we evaluated the intervention effect in the elderly and to analysed influencing factors.Methods1. Descriptive research. Through the report information management system of infectious disease and tuberculosis information management system, registered and reported medical records with tuberculosis patients from 2005 to 2013 in Dongchuan district had been collected and cleared up. To calculate incidence, mortality, the median delay and treatment success rate. To analyse influencing factors of treatment outcome among the elderly tuberculosis patients.2. Self-reflection community intervention study. Investigators entered into the elderly’s residence and conducted the first investigation. After the intervention, investigators conducted the second investigation. To calculate tuberculosis core knowledge awareness rate and full awareness rate in the first and second investigation. To evaluate the intervention effect, to analyse influencing factors. The statistic methods used in this study included statistic description, Chi-square test and unconditional Logistic regression.Result1. An average annual report incidence and mortality rates among the elderly tuberculosis patients from 2005 to 2013 in Dongchuan district were higher than others’ (p value<0.05). The diagnosis delay among the elderly thberculosis patients was better than others’.2. Receiving treatment rate and treatment success rate among the elderly tuberculosis patients were lower than others’(p value<0.05). The retreatment pulmonary tuberculosis patients (OR=4.433) and tuberculosis patients with non-implementation management system (OR=9.997) were the risk factors of the treatment outcome among the elderly tuberculosis patients.3. After the tuberculosis propaganda intervention, the awareness rate of tuberculosis among the elderly people was low (48.50%). Tuberculosis core knowledge among the elderly in the second investigation was better than the elderly’s in the first investigation(p value< 0.05). The biggest increase was tuberculosis symptom (73.41%), the minimum increase was where do you see a docor (13.84%).4. High-cultural level, workers, medical insurance for urban workers, tuberculosis contact and the elderly in Shaba and Xianghe communities, had a good tuberculosis intervention effect.ConclusionHigh prevalence, high mortality, low accepting treatment rates, low treatment success rate and low awareness rate, which made the elderly to be high-risk groups. Death and lose were the reason among the elderly tuberculosis patients. The disease classification and system management were the risk factors of the treatment outcome among the elderly tuberculosis patients. The propaganda intervention had effect on the elderly in Dongchuan district, we should enchance the propaganda. Culture, profession, medical insurance, tuberculosis contact and communities were influencing factors of the intervention effect. Tuberculosis propaganda should be long-term, sustainable, various and purposeful in order to satisfy different the elderly.
