Objective: On the basis of the previous research, this project use medical infrared thermal imaging technology to observe the moderating effect of abnormal infrared thermogram of CNBP model rats’ prostate body surface projection area by acupuncture at "Sanyin" acupoint, And analysis the application value of infrared thermal image technology in CNBP in clinical diagnosis and selecting the acupoint which tracking along meridian; Further immunohistochemical technique was used to study the effect mechanism of acupuncturing "Sanyin" acupoint in treat of CNBP from the perspective of COX-2 expressionthe in central(hypothalamus) and target organ(prostate), in order to provide basis for the clinical application of acupuncture treatment of CNBP.Methods: 60 SD rats(200-250 g, male) were randomly divided into blank group, grab group, model group, drug group, the "Sanyin" acupoint group and the sensitive acupoint area group by computer random number table method. Copying immunity CNBP model rats. Then, collecting the infrared thermogram of each rat-group’s prostate body surface projection area; Then, in accordance with the treatment requirements of each rat-group to give the treatment, 1 times a day, 10 days continuously. Gathering the infrared thermogram of each rat-group’s prostate body surface projection area again after treatment. And then using HE staining to observe the pathological morphology changing of each rat-group’s prostate tissue and hypothalamus organization. At the same time, using the immunohistochemical technique to observe the COX-2 expression of each rat-group’s prostate tissue and hypothalamus organization.Results:1 The pathological morphology changing of each rat-group’s prostate tissue and hypothalamus organization(1) The blank-group rats’ prostate tissue structure is normal; Grab-group rats’ prostate mesenchyme expand slightly, mesenchyme contains a small amount of fibre hyperplasia and inflammatory cell infiltration; A lot of glandular lumens are destroyed, within the interstitial fibers hyperplasia and a large number of inflammatory cells infiltration among model-group rats; The desturction of prostate tissue structure of "Sanyin" acupoint group, sensitive acupoint area group and drug-group are improved significantly compared with model-group, among these groups, the "Sanyin" acupoint group and drug-group’s improve-level are quite, and they are superior to sensitive acupoint area group.(2) Hypothalamus structure is normal among blank-group and grab-group, model-group rats’ hypothalamus tissues with mild hyperemia, nerve cells expand, nerve cells and blood vessels around the extracellular space widen obviously, and has a large number of inflammatory cells and microglia gather; The desturction of hypothalamus structure of "Sanyin" acupoint group, sensitive acupoint area group and drug-group are improved significantly compared with model-group, among these groups, "Sanyin" acupoint group and drug-group’s improve-level are quite, and they are superior to sensitive acupoint area group.2 The analysis of the infrared thermogram tracking along meridians among sensitive acupoint area groupCompared with the blank-group, the rats’(sensitive acupoint area group) emperature extracted from infrared thermogram rise abnormally in acupoint areas of spleen Meridian of Foot-Taiyin: Yinlingquan, Diji, Sanyinjiao and acupoint areas of conception vessel: Guanyuan, Zhongji, Qugu. All these acupoints are belong to sensitive acupoints of CNBP.3 The analysis of infrared thermogram of each rat-group’s prostate body surface projection regionBefore the treatment, compared with the blank-group, model-group, "Sanyin" acupoint group, sensitive acupoint area group and drug-group rats’ prostate body surface projection region have a significant rise in temperature, the both have the very significant difference(P<0.01). After treatment, compared with the blank-group, the temperature of grab-group rats’ prostate body surface projection area rises slightly, but there is no significant difference(P>0.05); Compared with model-group, three treatment group rats’ prostate body surface projection area all have different lower-degrees in temperature, the difference has the extremely significant difference(P<0.01); Comparison between three treatment groups, the level of the temperature of prostate body surface projection region are quite(P>0.05) among "Sanyin" acupoint group and drug-group’s, and they are superior to sensitive acupoint area group(P<0.01).4 The analysis of COX-2 expression in hypothalamus organization and prostate tissue of all group(1) Compared with the blank-group, grab- group rats have very small amounts expression of COX-2 in prostate tissue, and there was no significant difference(P>0.05); the expression of model-group rats’ COX-2 increase significantly in prostate tissue, it has very significant difference(P<0.01); Three treatment group compared with model-group, expression of COX-2 in prostate tissue reduce to some extent, it has very significant difference(P<0.01); Comparison between three treatment group, the COX-2 expression level are equal(P>0.05) among "Sanyin" acupoint group and drug group, and both are lower than sensitive acupoint area group(P<0.05).(2) Compared with the blank-group, grab-group have very small amounts of COX-2 expression in hypothalamus tissues, but there is no significant difference(P>0.05), the model-group rats’ expression of COX-2 significantly increased, it has a very significant difference(P<0.01); Three treatment groups compared with model-group, the expression of COX-2 in hypothalamus organizations all have different lower-degree, they have very significant difference(P<0.01); Comparison between three treatment groups, the COX-2 expression level of "Sanyin" acupoint group and drug group in hypothalamus tissues are equal(P>0.05), and lower than sensitive acupoint area group(P<0.05).5 The relationship analysis of prostate body surface projection area temperature, COX-2 expression of the prostate tissue and the hypothalamus among all groupsCompared with blank-group, the temperature of model-group rats’ prostate body surface projection area has a significant rise(P<0.01), while the COX-2 expression level of prostate tissue and the hypothalamus tissue also increased synchronously(P<0.01). After treatment, the abnormally elevated temperature of three treatment group rats’ prostate body surface projection area all have different lower-degrees(P<0.05), while abnormally high expression level of COX-2 of prostate tissue and hypothalamus organization also have synchronous reducing(P<0.05).Conclusion:(1) CNBP can cause abnormal high temperature of prostate body surface projection area, after acupuncture "Sanyin" acupoint, acupuncture sensitive acupoint area and prostat tablets lavage, the temperature reduces. Among them, the effective of acupuncture "Sanyin" acupoint and prostat tablets lavage are equal and both better than acupuncture sensitive acupoint area. Medical infrared thermal imaging technology has the significance in CNBP clinical diagnosis and guiding to choose point tracking along meridian, but it needs to further research and digging.(2) CNBP can cause unusually high expression of COX-2 in prostate tissue and hypothalamus organization, after acupuncture "Sanyin" acupoint, acupuncture sensitive acupoint area and prostat tablets lavage, both parts’ COX-2 expression are reduced, the effective of acupuncture "Sanyin" acupoint and prostat tablets lavage are equal and both better than acupuncture sensitive point area.(3) The temperature of prostate body surface projection area and the COX-2 expression level of prostate tissue and hypothalamus organization rise synchronously, suggests that there may be internal connection between the hypothalamus and prostate, COX-2 protein is one of the important factors that link. The mechanism of acupuncture "Sanyin" acupoint treating CNBP may be related to effective adjustment the COX-2 abnormal expression of prostate tissue and hypothalamus organization.
