Objective:To investigate the association of adiponectin level with premature coronary artery disease by measuring the serum APN level of premature coronary artery patients.Methods:108 hospitalized patients were enrolled in our study,who all underwent the coronary angiography from June 2012 to June 2014 in Department of Cardiology of Second Clinical Medcial College,Shanxi Medical University. All patients were divided into two groups by coronary angiography,premature coronary heart disease group(n=72) and control group(n=36).The clinical datas were collected between two groups.The serum concentrations of APN were measured by ELISA in all patients. All the research objects were grouped by clinical diagnosis,the number of coronary lesions,Gensini score,then used statistical software for analysis.Results:1.Compared with non-PCAD group,the proportions of male,smoking,hypertension,CAD family history,and the levels of alanine transaminase,aspartate transaminase,alkaline phosphatase,gamma glutamy1 transpeptidase, uric acid creatinine in PCAD group were significantly increased,the proportion of drinking in PCAD group decreased,while the levels of HDL, Apo A and APN in PCAD group were significantly decreased(P<0.05).There were no significant difference of the proportion of diabetes andthe levels of age,body mass index,systolic pressure,diastolic blood pressure,heart rate,blood glucose,CHOL,TG,LDL,Apo B between two groups.2.The logistic regression results revealed that the major risk factors of PCAD were CAD family history,smoking,low Apo A and APN level. 3.Serum adiponectin concentrations in patients with premature coronary heart disease were significantly lower than those in control group(P < 0.05).4.Patients in AMI group and UAP group and SAP group had lower serum adiponectin concentrations than those in control group(P < 0.05), no significant difference were found between UAP group and AMI group(P>0.05). Serum adiponectin concentrations were decreased in turn in those groups(P<0.05). 5.Patients in single vessel disease group,double vessels disease group and multiple vesselsdisease group had lower serum adiponectin concentrations than those in control group(P<0.05).Serum adiponectin concentrations were significantly decreased in turn among single vessel disease group,double vessels disease group and multiple vessels disease group(P< 0.05). 6.Grouped by Gensini score,the serum APN level in light stenosis group,moderate stenosis group and severe stenosis group had lower serum adiponectin concentrations than those in control group(P<0.05). Serum adiponectin concentrations were significantly decreased in turn among light stenosis group, moderate stenosis group and severe stenosis group(P<0.05).Conclusion:1.The present suggested that CAD family history, smoking and low Apo A and APN level may be the major risk factors of PCAD. 2.APN were associated with the degree of coronary lesion in patients with PCAD and hypoadiponectinemia could promote the formation of arteriosclerosis plaque and development.
