Clinical Efficacy Study On The Treatment Of Femoral Intertrochanteric Fractures With Anatomical Dynamic Hip Locking Plate Combined With Traditional Chinese Medicine Constitution

Objective: By the TCM Constitution Classification Observe the different constitution of femoral intertrochanteric fracture patients in anatomical dynamic hip locking plate internal fixation clinical efficacy and postoperative complications of the study, a preliminary estimate of the prognosis of the patient’s constitution is different and the formulation of prevention and treatment of postoperative complications of measures;according to the patient’s constitution is different to explore individualized treatment of femoral intertrochanteric fracture of improve the cure rate and reduce the complications is of great significance.Methods: The clinic in Yanan Hospital of Kunming city in accordance with the inclusion criteria of femoral intertrochanteric fracture between the "Chinese constitution classification and decision" standard, into the Normal constitution group and Qi deficiency group were 33 cases. This study will meet the inclusion criteria and exclusion criteria were the use of anatomical type dynamic hip locking plate fixation by research group guidance teacher unified surgical treatment and postoperative patients in each group received postoperative conventional swelling and pain relief, prevent thrombosis,prevent infection etc. basic treatment, patients in two groups were observed after healing and curative effect. At the same time, in order to ensure the comparability of patients after treatment in the case of a patient’s choice, the balance between the groups on the age,gender, and the injury mechanism of the fracture classification was also noted. The factors that affect the treatment outcome of patients are in a state of equilibrium.Results:(1) This study showed that by the tutor team of independent design, research and development of the anatomical type dynamic hip locking plate fixation for the treatment of AO type A1.1 to A2.1 relatively stable type of intertrochanteric fracture with definite curative effect, The excellent and good rate was 95%, and the excellent rate of qi deficiency group was 84.6%. The total excellent and good rate was 90.9%.(2) Compared with the gentleness group and Qi deficiency group of patients after 3 days of pain score: the gentleness group(7.015 + 0.206) Qi deficiency group(7.931 + 0.395)by independent sample t test(t=-8.753, P=0.00 < 0.01) there was a significant difference between the statistical results, the difference was statistically significant; the results of the7 day postoperative pain score the two group: the gentleness group(7.420 + 0.244) Qi deficiency group(8.615 + 0.547) is shown in(t=-8.591, P=0.00 < 0.01) there was a significant difference between the statistical results, the difference was statistically significant; 14 days the pain scores were compared in two groups of patients, the gentleness group(6.180 + 0.132) Qi deficiency group(6.277 + 0.136), statistical results(t=-2.033, P=0.051 > 0.05) no significant difference comparison, the results were not statistically significant.(3) After first months, second months, third months, the patients with Qi deficiency group and the quality group Harris hip function score comparison :The first months: flat and quality group(83.40 + 2.32) Qi deficiency group(76.23 + 2.71), by independent sample t test(t=8.280, P = 0.00 < 0.01); the second months: flat and quality group(90.15+ 388) Qi deficiency group(81.15 + 2.76) by t test(t=7.229, P = 0.00 < 0.01) contrast differences, statistical results were statistically significant; the third months: flat and quality group(93.20 + 4.24) Qi deficiency group(87.69 + 4.63) by t-test(t=3.520, P =0.001 < 0.01) contrast differences statistic result has statistical significance.(4) Comparison of two groups of patients with concurrent rate,the Qi deficiency group was 38.3%,But the Normal constitution group was 5%.Conclusion:(1) This study showed that by the tutor team of independent design, research and development of the anatomical type dynamic hip locking plate fixation for the treatment of AO type A1.1 to A2.1 relatively stable type of intertrochanteric fracture with definite curative effect.(2) Qi deficiency of femoral intertrochanteric fracture patients with flat and quality in patients with postoperative pain in the postoperative 3 days and postoperative 1 week grading results show that two groups of patients in pain intensity on flat and vitrinite pain tolerance is better than that of qi deficiency group; postoperative 14 days results showed that pain intensity of two groups was no significant difference; postoperative hip function recovery flat and quality group was significantly better than the Qi deficiency group; the incidence of postoperative complications was Qi deficiency quality group is higher than that of flat and quality group; flat and quality average down earlier in Qi deficiency group;the sick average hospitalization days Qi deficiency group leader Yu Ping and quality group.(3) Flat and quality groups of patients in Qi deficiency patients in curative effect and prognosis of differences; therefore, according to the different physical, estimated and the prognosis in patients with postoperative complications, clinical individualized treatment of femoral intertrochanteric fractures provide new treatment options.
