| BackgroundAs one of the most important treatment method of Chinese medicine, tuina therapy has been widely used in clinical treatment of various diseases. At present, more and more Chinese medicine scholars are researching on tuina, and their studies mainly focus on the effect of tuina on the clinical disease. However, it’s less involved in tuina’s methods or mentioned the influence of stimulus quantity to treat disease. The operation of tuina itself involves the treatment time, power, frequency and other elements. Effective tuina depends on 4 conditions:certain stimulation, operation time, suitable frequency and right direction. Therefore, we need to make the operation and stimulus quantity of tuina specific and standard by further research.ObjectiveAs above said, effective tuina depends on 4 conditions:certain stimulation, operation time, right frequency and direction. To observe the clinical efficiency of tuina with different frequency but same technique, the research used different frequency pushing manipulation with One-finger meditation to treat spleen and stomach deficiency-cold type of epigastralgia under the control of the same force and stimulate the premise stimulation time. Patient outcomes through comparison between different groups best summed up pushing manipulation with One-finger meditation of frequency bands, the effective frequency band frequencies and invalid. The study pave the way for the standardization of TCM tuina therapy improving the efficiency of treatment, helping TCM tuina combined with modern and promotion.MethodsThe spleen and stomach deficiency-cold type of epigastralgia patients in line with the study design of this thesis were originally collected from the Out-patient department of Xianliedong and the No. lAffiliated Hospital of Guangzhou University of Chinese Medicine. There were 80 cases in total, and randomly split into A, B, C, D, four groups of 20. Using the symptoms score list of the spleen and stomach deficiency-cold type of epigastralgia patients and VAS scale to observe the changes of improvements in patients with pain and other symptoms before and after treatment. The clinical therapeutic effect was assessed by the spleen and stomach deficiency-cold type of epigastralgia symptoms score list and VAS scale before and after treatment. Then statistically analyzed all result data with SPSS 22.0 software, in which the measurement data were used on t-test; ranked data were analyzed with rank sum test to observe whether there were differences of both treatment results.ResuIts1. Comparing the before and after symptoms scores of the spleen and stomach deficiency-cold type of epigastralgia patients’we can figure out that the 4 groups of patients have varies degrees of change relative to pre-treatment (P <0.01).2. Recovering comparison on "before and after" with VSA scale:the 4 groups of patients with epigastric pain eased in varies of degrees.3. Comparison of clinical efficacy:The total effective rate of group A was 100%, the group B’s was 83.33%., the group C s was 90%and the group D’ s 20%. Four groups are obviously different. (P>0.05).4. Recovering comparison on " before and after " with treatment rates, improve value,the spleen and stomach deficiency-cold type of epigastralgia symptoms scores and the VAS scale:treatment of group Bis better than the other three groups, group D is the worst. So the frequency of group B is the optimal frequency of treatment, i. e.,100-150 times/min. Group A-and C have a therapeutic effect,but the difference compared with group B, is an effective means of treatment frequency of pushing manipulation with One-finger meditation rather than the best treatment frequency. And group D 200-250 times/min is noneffective frequency band.ConclusionThrough clinical observation and studies, we have the following conclusions:101-150 times/min, the frequency of group B is the optimal frequency of treatment for the the spleen and stomach (weak) type epigastralgia, while group A’s 50-101 times/min and group C s 151-200times/min are both therapeutic effective frequency. Group D’s frequency,201-250 times/min, is noneffective frequency. |
