| Objectives:1.Summarize the main academic ideas and clinical experience in the treatment of hypertension by Professor Liu Dehuan.2.Based on the summary of professor’s clinical experience for treating hypertension, adopt ambulatory blood pressure (ABPM) to monitor and calculate the ratio of blood pressure trough to peak (T/P) and smoothness index (SI) as the main indicator, observe and evaluate the antihypertensive effect and efficacy of clinical symptoms by the professor’s empirical formula " Yu Zhuo Qing Granule " and amlodipine for treating two hypertensive patients (phlegm, liver and kidney).Methods:1.Summary of academic experience:through three-year’s theoretical study and clinical practice, systematically and comprehensively summarize the professor’s academic thoughts and clinical experience, look for an entry point from the theoretical origins, pathogenesis, syndrome differentiation, prescription drugs, explore the professor’s academic thought, analyze the trajectory of his academic thought, and finally summarizes the essence of the professor’s academic thought. In the meanwhile, think deeply based on the summary of academic thoughts with the combination of their own views and ideas during clinical practice, to deepen and supplement the professor’s academic thoughts.2.Clinical studies:randomly divide the 60 eligible patients into two groups, namely the observation group and the control group,30 cases in each group. Both groups were given amlodipine besylate antihypertensive treatment, and the observation group was furthered given the traditional Chinese medicine prescription stasis turbid Granule on this basis; treatment for 12 weeks. Observe the ambulatory blood pressure before and after 24 hours of the test, calculate the ratios of blood pressure trough to peak (T/P) and smoothness index (SI), and compare the clinical efficacy of patients with the antihypertensive efficacy of TCM symptoms.Result:1.Summary of academic experience:The Hua Yu Zhuo Yi Gan Shen thought is a therapeutical principle for hypertension disease, raised by Professor Liu Dehuan in Quanzhou Hospital of Traditional Chinese Medicine, on the basis of his rich knowledge and experienced clinical skills, according to traditional Chinese medicine theory. Hua Yu Zhuo Yi Gan Shen, namely removing blood stasis-turbid, and tonifying liver and kidney. The Hua Yu Zhuo Yi Gan Shen thought integrates the correlated theory of Phlegm and Stasis, Blood Stasis theory, Homogeny of Clear fluid and Blood thought and Homogeny of Liver and Kidney thought, and then the modern medical theory and pharmacological research are well applied to its prescriptions. The Hua Yu Zhuo Yi Gan Shen thought has the full theory basis on the treatment of TCM Heart Disease,focusing regulate the body’s overall function, and Conducive to the smooth blood pressure in hypertensive patients.2. Clinical studies:(1) After treatment,24 ambulatory blood pressure decreased in both groups,and the decrease of 24hSBP,24hDBP in the treated group was superior to that in the control group (P<0.01). (2) The smoothness index for SBP and DBP in treated group were significantly higher than those in control group(P<0.01), and trough to peak ratios in treated group were also higher than those in control group,but no statistical difference between them (P>0.05). (3) After treatment,The total efficiency of the clinical efficacy of antihypertensive in the treatment group was 93.3% and that in the control group was90.0%. The treatment group was superior to the control group (P<0.001). (4) After treatment,The total effective rates of TCM syndromes in the treatment group was 90.0% and that in the control group was 76.7%. The treatment group was superior to the control group (P<0.001).Conclusion:1. The Hua Yu Zhuo Yi Gan Shen thought,raised by Professor Liu Dehuan for the therapeutical principle of hypertension disease, integrates the correlated theory of Phlegm and Stasis, Blood Stasis theory, Homogeny of Clear fluid and Blood thought and Homogeny of Liver and Kidney thought, and then the modern medical theory and pharmacological research are well applied to its prescriptions. This embodies the thought of TCM Modernization, and has innovation significance in theory and clinical value in practice.2. The composing prescription of Yu Zhuo Qing Granule from the Hua Yu Zhuo Yi Gan Shen thought, with the reinforcement and elimination in combination, application of both cold and heat, and landing and taking off in an orderly manner, possesses the function of removing blood stasis-turbid and tonifying the liver and kidney. There was a clear structure monarch in the composing prescriptions,which can focuse on regulating the balance of body enginery,thus focuses the body’s overall function and improves the clinic sympotums in patients.3. On the basis of western medicine on blood pressure, the use of Chinese medicine prescription of Yuzhuoqing granule can make blood pressure decrease to normal and further controle the blood pressure variability in patients with hypertension wiht phlegm accumulates with stagnant blood syndrome and Yin deficiency of liver and kidney syndrome,Which has positive effect for the prevention on target-organ damage of hypertension. |
