Objective:To investigate TCM syndrome characteristics and quality of life (QOL) about diarrhea-predominant irritable bowel syndrome (IBS-D), to provide a reference for the prevention and treatment of IBS-D.Methods:This study took the outpatients with IBS-D as the research object which saw a doctor in Wangjing hospital from January 2015 to March 2016.The research group collected and standardized the information of patients With the method of questionnaire. To investigate the distribution characteristics of age, gender, literacy, course of disease, symptoms, in different TCM syndrome differentiations, and to illustrate the influence factor of QOL.Results:Of 64 patients with IBS-D, female ratio is about 1.2:1; mainly of young and middle-people; Above college culture level, mainly engaged in non-manual work; The course of disease is generally longer, wherein 1-5 years is Maximum, accounting for 59.4%.The main predisposing factors are diet and emotions, followed by climate and environment, it can be a single factor or multifactorial combined effects.Patients with IBS-D major abdominal symptoms was bellyache (96.9%) borborygmus (90.6%) and Bloating (71.9%), most of them with defecation urgency (87.5%), defecation tension(65.63%)and Stool not the flu(76.56%).The moderate score of irritable bowel syndrome-severity scoring system (IBS-SSS) accounted for 64.1%.Digestive symptoms are more obvious, mainly for abdominal fullness (60.9%), distending pain in hypochondrium (56.3%), inappetence (51.6%) belching (50.0%); Systemic symptoms are often accompanied by depression or irritability (76.9%), thirsty but drink less (59.4%) and so on.TCM Syndrome Differentiation Typing in liver depression and spleen deficiency syndrome is the most common (46.9%), second is Spleen deficiency wet-sheng syndrome (29.7%) and the last is spleen-kidney yang deficiency syndrome (23.4%).Wherein liver depression and spleen deficiency syndrome in 21-30 years and 31-40 years old groups are most common, spleen deficiency wet-sheng syndrome in the majority with 41-50 years old groups.IBS-D patients eight dimensions of IBS-QOL scores were lower than in healthy people (P<0.05), especially decreased in Dysphoria (DP)ã€Health Worry (HW) than normal.Gender and quality of life was no correlation on different dimensions scores (P>0.05); Except Dysphoria (DP), there were no significant differences in each age groups (P>0.05); there was no correlation between the course and IBS-QOL different dimensions scores (P>0.05);there was no correlation between the course of disease and literacy (P>0.05); there was a negative correlation among each IBS-SSS groups on four dimensions scores (Dysphoria, Health Worry, Relationship and Body Image)(P< 0.05); Three TCM syndrome differentiation types all have significantly difference during three point of IBS-QOL (Dysphoria, Food Avoidance, Relationship) (P<0.01).Liver depression and spleen deficiency syndrome in three aspects (Dysphoria, Health Worry, relationship) have the lowest points; Spleen deficiency wet-sheng syndrome in Food Avoidance have lower points; spleen-kidney yang deficiency syndrome have the lowest points in Health Worries (HW) Conclusion:IBS-D is a functional gastrointestinal disorders disease, the mainly incidence of crowd of IBS is more common young and middle-aged whose aged 21-50 years and high-cultural level; Longer course, but the majority of the severity of symptoms in patients with moderate majority, the most common type is Liver depression and spleen deficiency syndrome; Quality of life in patients with IBS-D is lower than the healthy population, which often influenced by age, severity of symptoms and other factors; the gender, literacy and course of disease of less impact on the quality of life of patients. |
