Objective To study the prevalence of metabolic syndrome(Met S) and its components, and analyze its risk factors among urban inhabitants(More than 5 years) aged 20-74 years old in Lanzhou, Gansu province, comparing the differences among different diagnostic criterions for MetS. Methods A stratified, cluster and random sampling des ign was used to select participants who were 20-74 years old living in their local residence for at least 5 years, From September 2013 to December 2014. There are 2996 participants in our study. All subjects firs tly completed a questionnaires survey, including basic characteristics, occupation, education level, family income, diet, smoking, drinking and exercise, various kinds of chronic diseases history. Immediately, these participants followed a physical examination, including Height, weight, Waist circumference, Hip circumference, heart rate and blood pressure and laboratory tests, including fasting plasma glucose and blood lipid. The Prevalence of MetS and its components among 20-74 years old adults in Lanzhou city was assessed according to different diagnostic criterions, namely Joint Committee for Developing Chinese Guidelines(JCDCG) in 2007, the international diabetes federation(IDF) in 2005, the American heart association(AHA) revis ion of the project group national cholesterol Ⅲ(ATPⅢ) in 2005, the international society for more than a joint statement(JIS) in 2009. We use binomial logistic regression analys is methods to analyze the risk factors of MetS, using the Kappa value and ROC curve to compare the differences and consistency of four different diagnostic criterions. Results The prevalence and standardized prevalence of MetS were 22.0%(16.3%), 34.0%(30.9%), 37.4%(34.1%)and 37.7%(34.3%),(P<0.05), respectively, according to the JCDCG, IDF, ATPⅢ, JIS. A binomial logistic regression analysis showed that the region urban, men, smoking, the history of diabetes, high blood pressure and high cholesterol, body mass index(BMI) and middle-aged and elder ly adults(over 40 years)(P<0.05) were associated with the MetS. The highest consistency of JIS, ATPⅢ and IDF for the diagnosis of Met S was 99.7% and 96.6%, respectively, with their Kappa value being 0.99 and 0.93, respectively. Therefore, the concordance between JIS and ATPⅢ is the highest. Conclusions According to four kinds of diagnostic criteria, there are differences MetS and its components detection among 20-74 years residents in Lanzhou, Gansu province. JIS criteria is better than any other criteria in screening for MetS and its abnormal components. The prevalence of Met S in Lanzhou city is high and has become a public health problem requir ing urgent attention for prevention and treatment. In view of Lanzhou being the more developed economy regions, we should take positive measures to reduce the cardiovascular disease risks, especially, among older men and urban residents as high risk group of MetS.
