Objective:(1). Discussion the relationship between the activation level of NF-κB(nuclear factor-κB) p65 in PBMC(peripheral blood mononuclear cells), plasma MCP-1(monocyte chemoattractant protein-1), s CD40L(soluble CD40 ligand) concentration and cerebral infarction;(2). Investigating the correlation of those arteriosclerosis associated inflammatory factors NF-κB, MCP-1 and s CD40 L to explore possible intermediary signal pathway;(3). Exploring the relationship between CD40-1C/T gene polymorphism and NF-κB p65, s CD40 L, MCP-1 levels to speculate the inflammatory signaling pathways between CD40-1 SNP and atherosclerosis. Methods:Selecting 40 cases of acute cerebral infarction patients met the inclusion criteria as the Cerebral Infarction group, and the same period 40 cases of medical examination center without the history of stroke in healthy people as a Control group. Using immunocytochemistry assay studied the PBMC NF-κB p65 protein positive rate. The plasma MCP-1 and s CD40 L concentration was detected by ELISA assay. Detected CD40-1 SNP genotypes by Taq Man probe technology. Quantitative data were normally distributed between the two groups were compared using t-test. Rates analyzed by the Chi-square test. Using the Pearson correlation test correlation analysis, α = 0.05. Results:(1)Infarction group: PBMC NF-κB p65 positive rate was(30.13±15.86)%.The plasma concentration of MCP-1 was(260.34±18.23) pg/ml, s CD40 L concentration was(71.17±25.56) pg/ml. Control group : NF- κB p65 positive rate in PBMC was(17.58±5.04)%.The plasma concentration of MCP-1 was(246.08±11.40) pg/ml, s CD40 L concentration was(57.15±21.91) pg/ml.The NF-κB p65 positive rate and plasma MCP- 1, s CD40 L concentration of Cerebral infarction group were significantly higher than Control group, the difference was statistically significant(P<0.05).(2)In all samples: There is a positive correlation between PBMC NF-κB p65 positive rate and MCP-1 level( r=0.473, P <0.001); There is a positive correlation between MCP-1 and s CD40 L level( r=0.252,P=0.026).There is no significant correlation between PBMC NF-κB p65 positive rate and s CD40 L level(P=0.179). In Cerebral infarction subgroup: There is a positive correlation between PBMC NF-κB p65 positive rate and plasma MCP-1 levels(r=0.391,P=0.015); There is a positive correlation between MCP-1 and s CD40 L levels(r=0.252, P=0.026). The Control group subgroups: There is no significant correlation between NF-κB p65 positive and plasma MCP-1 levels,P>0.05; There has no significant correlation between MCP-1 and s CD40 L levels,P>0.05.(3)The NF-κB p65 positive rate of CD40-1 SNP genotype CC, CT and TT was(27.07±17.50)%,(21.29±7.84)% and(18.30±7.82)%,respectively. The plasma MCP-1 concentrations of CD40-1 SNP genotype CC, CT and TT was( 259.55±19.36)pg/ml,(256.11±12.01)pg/ml,(251.32±20.32) pg/ml, respectively. The s CD40 L plasma concentrations of CD40-1 SNP genotype CC, CT and TT was(69.76±25.61) pg/ml(62.67±21.34) pg/ml(57.00±28.29) pg/ml, the difference was no statistically significant(P>0.05), respectively. But there has obvious concentration gradient. By stratified analysis, The difference of Control subgroup and Cerebral infarction subgroup of CC,CT and TT genotype type in NF-κB p65 positive rate, MCP-1 and s CD40 L concentration was no statistically significant(P>0.05), respectively. But there also has concentration gradient. Great atherosclerotic cerebral infarction group: There has no significant difference between three genotypes at positive rate of NF-κB p65, plasma MCP-1 and s CD40 L concentration(P>0.05), but there are also obvious concentration gradient. Conclusion:(1)NF-κB p65 activation level in PBMC and plasma MCP-1, s CD40 L levels were significantly associated with cerebral infarction.(2)s CD40 L, NF-κB, MCP-1 may be related to inflammatory signaling pathways downstream on inflammatory factors. s CD40 L prefer to activate NF-κB signaling pathway of non-classical.(3)Yet we have not considered there is a relationship between CD40-1C/T gene polymorphism and the positive rate of NF-κB p65, the concentration of plasma MCP-1 and s CD40 L.
