Objective:Because of its oxidation and dual antioxidant properties, uric acid as a metabolite of purines, has been widespread attention in recent years, and has been became the focus of much research content. On the one hand, uric acid was involved in inflammation, oxidative stress, immune response and promots lipid peroxidation; on the other hand, uric acid as a powerful antioxidant that could be rid of about 66.7% of oxygen free radicals and inhibit lipid peroxidation in the body. Carotid atherosclerosis as a risk factor for ischemic stroke, has been widely recognized. The occurrence and development of carotid atheroscler were very complexly pathophysiological process, involving many aspects of oxidative stress, inflammation, immune response and so on.spot block vulnerability and development of atherosclerotic arterial sclerosis lesions related, such as lipid accumulation, proteolysis, apoptosis, thrombosis and vascular endothelial cell injury. low-density lipoprotein oxidation as oxidative stress, inflammatory reaction product formed through the carotid artery atherosclerotic plaque in. When brain tissue ischemia, acute ischemic stroke lesion was central necrosis area and its surrounding ischemic penumbra composition, when there was a blood supply and necrosis ischemic penumbra region, oxygen entered ischemia organization, then a large number of generating free radicals, so that the body oxidation and antioxidant mechanisms lose their balance, and the body to break the dynamic balance of radical generation scavenging system, resulting in oxidative stress, and neuronal damage caused by defects in the body’s functions. Uric acid involved in atherosclerosis, particularly in the acute ischemic stroke has been controversial. The main purpose of this study were in the process of formation of uric acid in the role of carotid atherosclerosis, and to explorate uric acid in patients with acute ischemic stroke mechanism.Methods:165 hospitalized acute ischemic stroke patients were closed in the observation group and 180 cases for the same period of examination population were closed in control group. Recording age, sex, hypertension, diabetes, coronary heart disease, smoking(average daily smoking> 1), drinking(alcohol or average daily alcohol consumPtion is greater than or equal to 50 gram) and other general information. To measure the concentration of serum uric acid, low-density lipid protein,high-density lipoprotein, oxidized low-density lipoprotein, triglycerides and total cholesterol. Color doppler ultrasound carotid plaque, according to the nature of the plaque into without plaque, stable plague group and unstable plaque group, and counted; between observation group and the control group and the group of uric acid group, differences between lipid levels. Observation group used by the US National Institutes of Health Stroke Scale(NIHSS) neurologic impairment degree score to evaluate the correlation between uric acid, oxidized low density lipoprotein and NIHSS score.Results:1. Observe carotid atherosclerotic plaque detection rate was higher(P<0.05), and unstable plaque detection rate in stable plaque group(P<0.05);2. Uric acid levels were higher than the observation group, while oxLDL levels were lower than the control group(P <0.05);3. Observation group increased plaque instability, elevated uric acid levels, oxLDL levels decreased(P <0.05);4. The observation group uric acid and oxidized LDL levels were negatively correlated(P <0.05);5. Observe the group uric acid levels were negatively correlated with the NIHSS score, that with increasing uric acid levels, NIHSS scores decreased.6. The observation group HDL levels lower than the control group, while low-density lipoprotein, total cholesterol levels were higher. Conclusions:In the acute phase of ischemic stroke uric acid, carotid atherosclerosis development, and with atherosclerotic plaque instability associated. Atherosclerotic plaque was a risk factor for ischemic stroke, and unstable plaque relationship with ischemic stroke more closely. Uric acid and acute ischemic stroke patients nerve function impairment related to its antioxidant uric acid played a role in acute ischemic stroke, inhibits oxLDL generate, protect the brain.
