Objective1) To study the prevalence of obstructive sleep apnea-hypopnea syndrome(OSAHS) aged over 20 years in Putuo district of Shanghai.2) To investigate the risk factors of OSAHS and provide guidance of the primary prevention for OSAHS.Method1) Questionnaires were surveyed to the staff of enterprises and institutions in Putuo district who come to the medical, the questionnaire covering essential information(age, gender, height, weight),whether snoring, affect others, the frequency of snoring(apnea), history of hypertension, smoking,drinking, whether have fatigue in early morning or during the daytime,daytime sleepiness scale (ESS).2) Then divided the questionnaires into two groups according to the severity of snoring(group 1 were questionnaires answered never, rarely or mild snoring, group 2 answered moderate or severe snoring),then randomly selected 100 cases from each group to line overnight polysomnography(PSG) to estimated the prevalence of OSAHS.Results1)Among the 8000 questionnaires issued,7180 subject(89.8%) had answered the questionnaires, and among them 6842(95.3%) were available.The mean age of the respondents was(48.3±14.0) years, body mass index (BMI) (23.5±6.1) kg/m2, sleep time(7.0±1.2) hours. Questionnaires were divided into five levels according to snoring frequency from low to high:never snoring, occasionally (1-2 times per month), mild (1-2 times per week), moderate (3-4 times per week) and severe (everyday).In those 6740 persons surveyed,3811 persons (55.7%)answered never snoring,222 (3.2%) occasionally snorers,804(11.8%)mild snorers,582 (8.5%)moderate snorers,1423 (20.8%)severe snorers. The proportion of severe snoring increasing with age before 70 years,41.7% persons of the 60-69 age group had moderate to severe snoring;The proportion of severe snoring reduced with age from 70 years on.2)We discovered that the high risk factors of snoring were gender,weight,BMI,history of hypertension,drinking and smoking. The ESS score was significantly different by different degrees of snores, the ESS score of moderate and severe snoring group was significantly higher than mild, occasionally or never snoring group (p<0.01). By the standard that AHI≥ 5 times/h and the lowest oxygen (LSaO2≤0.90, ESS score≥9 points, the prevalence of OSAHS was 5.10%. There were significant differences in gender,weight,BMI,drinking and smoking between people with OSAHS and non-OSAHS.3)According to the results of the questionnaires and PSG result,there were 1.4% persons with OSAHS in the group which answered never snoring, occasionally snoring, and mild snoring or do not know whether snore.Conclusion1) The prevalence of obstructive sleep apnea-hypopnea syndrome in Chinese adults aged over 20 year in Putuo district of Shanghai was 5.10%(male6.23%, female3.88%).2) The risks factors of snoring and OSAHS were gender,weight,BMI,history of hypertension,drinking and smoking.3) There were OSAHS patients in those persons who answered never, occasional, mild snoring and not clear whether snoring,and they were easily missed diagnosis,so our clinical workers should pay attention to early diagnosis and treatment OSAHS of them.
