Backgrounds:AIDS and syphilis are two important infectious diseases that seriously endanger human heath.People suffering from AIDS is due to human immunodeficiency virus(HIV)infection,and the mortality is very high among AIDS patients.Since the first case of AIDS was found in China in 1985,the rate of HIV infection has been increasing by 30%every year,and the epidemic is spreading from high-risk groups to the general population.On the other hand,people who are infected with Treponema pallidum will suffer from a systemic,chronic,sexually transmitted diseases-syphilis,which will make the body's multi-system,multiple organs being damaged,with the emergence of chancre as a representative clinical manifestations,making the organization function damaged,and the effect is even life-threatening.F Since 1980s,the number of cases of syphilis in China has risen sharply,and the situation is becoming more and more serious.Hospital is the first place where AIDS and syphilis patients are diagnosed and treated.The level of knowledge about AIDS and syphilis among health care workers can affect not only the treatment,but also precision of disease report as well as estimation of disease burden.With the rapid development of medical technology,the progress of human society and the opening trend of people's thinking,there are more and more diagnostic and therapeutic techniques including the use of invasive operations.The possibility of various occupational exposure are increasing in the medical staff,threatening their physical and mental health and becoming one of the most serious public health problems.Hospital is a place where patients concentrate,the risk of various diseases increases in hospitals.Health care workers are persons with the most contact with the patient,so there is a high risk of occupational exposure among them.Objectives:To understand the knowledge of AIDS and syphilis as well as the corresponding occupational protection among medical staff in Zaozhuang City,so as to find existing problems,propose solutions,provide background knowledge and theoretical basis for future vocational training of occupational protection among medical staff.Methods:We selected doctors,nurses,and technical personnel 9 hospitals of Zaozhuang city,including 2 tertiary hospitals,3 secondary hospitals and 4 community health service centers.and hospitals,using self-administered questionnaires survey.The self-administered questionnaire was used to collect information and the data were then processed using Excel.Chi square test and Spearman correlation were used to analyze the relationship between the characteristics and the knowledge about the AIDS/syphilis exposure in univariate analysis.After screening out the variables with statistical significance,multiple Logistic regression was used to further clarify the factors influencing occupational exposure knowledge.Data analysis were conducted using SPSS19.0 statistical software,and test level was set to P<0.05.Results:The medical staffs knowledge about the basic knowledge of syphilis and HIV and the knowledge of occupational exposure were divided into three levels:poor,moderate and good.Among the investigated medical staff,153(64.29%)had a good knowledge of the occupational exposure knowledge of HIV,58(24.37%)had moderate knowledge,and 27(11.34%)had poor knowledge.For syphilis,39(16.38%)had poor knowledge,38(15.97%)and 161(67.65%)had moderate and good knowledge,respectively.In the multivariate logistic analysis of the knowledge of HIV occupational exposure in the medical staff,using tertiary hospital as the reference,staff in secondary hospitals(P<0.001)and primary hospitals(P=0.005)had significant lower knowledge.Using others as reference,doctors(P<0.001),nurses(P =0.001),and laboratory staff(P<0.001)had significant higher knowledge.The doctors' knowledge is 2.91 times of other staff,nurses' knowledge is 4.109 times and lab staffs knowledge is 9.51 times of other staff.Compared with staff with higher professional title,those with lower title had lower knowledge level(P ?0.007).In the multivariate logistic analysis of the knowledge of syphilis occupational exposure in the medical staff,using others as reference,doctors(P<0.001),nurses(P = 0.002),laboratory staff(P=0.010)had significant higher knowledge.The doctors' knowledge is 5.504 times of other staff,nurses' knowledge is 6.163 times and lab staffs knowledge is 5.62 times of other staff.Compared with staff with higher professional title,those with lower title had lower knowledge level(P<0.001).Conclusions:Medical staff in tertiary hospitals had a better knowledge of HIV and syphilis occupational exposure.However,there were also problems,such as understanding is not comprehensive,especially among staff who do not work in clinical position and those with lower professional title.There is a general lack of regular training in the secondary hospitals and community service centers,where medical staff generally lacks knowledge about the knowledge and protection of occupational exposure and lacks expertise in this area.Hospital leaders and relevant training departments should pay attention to this.
