Objective: After knowing about testicular torsion in earlier stage of rats’ sexual maturity,it is necessary to study what kinds of surgical procedures(reset/resection)should be applied to different states of testicular torsion(torsion of non-necrotic testis,torsion of suspected necrotic testis,and torsion of necrotic testis),especially for torsion of suspected necrotic testis and torsion of necrotic testis),so as to reduce damage of sexual maturity on testicular spermatogenic function.Methods:(1)Experimental method: Internationally common Turner method was applied to produce the testicular torsion model in early stage of sexual maturity;56 SD rats of three weeks old were divided into seven groups.Every group included 8 rats.A group was selected as a control group to conduct sham operation at random.Torsion of non-necrotic testis reset group: left testis was rotated in counterclockwise for 90° and testis was reset after fixing 4h;torsion of non-necrotic testis excision group: left testis was rotated in counterclockwise for 90° and testis was excised after fixing 4h;torsion of suspected necrotic testis reset group: left testis was rotated in counterclockwise for 360° and testis was reset after fixing 4h;torsion of suspected necrotic testis excision group: left testis was rotated in counterclockwise for 360° and testis was excised after fixing 4h;torsion of necrotic testis reset group: left testis was rotated in counterclockwise for 720°and testis was excised after fixing 4h;torsion of necrotic testis excision group: left testis was rotated in counterclockwise for 720° and testis was excised after fixing 4h.After operation,rats were continued being fed for 6 weeks until they reach the sexual maturity.The rapid de-cervical vertebra method was used to execute SD rats.Offside testis was cut off to observe tissue morphology under light microscope with paraffin embedding,detection of stem cell factor receptor c-kit by immunohistochemical detection,and expression of phosphoinositide 3-kinase PI3 K.Meanwhile,heart blood was centrifuged to take serum.ELISA was used to detect gonadal hormone concentration(follicle-stimulating hormone FSH,luteinizing hormone LH and testosterone T).(2)Statistical method: Excel was used to record experimental data.SPSS18.0 was applied to finish data analysis.`X ± S was used to express measurement data.Single-factor analysis was used to compare indicator difference between groups.If there is statistical significance,LSD method should be further applied to make a pairwise comparison.With P<0.05,it showed that there was statistical significance.Results:(1)Changes of histomorphology: seminiferous epithelium of testis on uninjured side in the control group was complete and no falling off.Spermatogenic cells at different levels were arranged orderly.There was no shrinking seminiferous tubule.In torsion of non-necrotic testis reset group and excision group: there was no obvious morphological change in testis on uninjured side;In torsion of suspected necrotic testis reset group and excision group: light morphological changes were mainly reflected in 1-2 layers of reduction in seminiferous epithelium cell layers.Some germ cells might fall in lumen.In torsion of necrotic testis reset group and excision group: cell layers of testis’ s seminiferous epithelium were reduced by 3-4 layers or 5-6 years.Some of them fell in the lumen.Germ cells were arranged chaotic,showing atrophy of seminiferous tubule.There was no obvious morphological change in the later.(2)c-kit and PI3 K expression of spermatogonium in testicular tissue: c-kit and PI3 K proteins in testicular tissue of uninjured side in the control group were presented in strong positive expression in cell membrane and cytoplasm of spermatogonium;c-kit and PI3 K proteins in testicular tissue of injured side in torsion of non-necrotic testis reset group and excision group were presented in positive expression;In torsion suspected necrotic testis reset group and excision group: cell membrane and cytoplasm of spermatogonium were presented in yellowish-brown and positive expression.In torsion necrotic testis reset group and excision group,c-kit and PI3 K in cell membrane and cytoplasm of spermatogonium was presented in weak positive,while cell membrane and cytoplas m in later spermatogonium were presented in yellowish brown and storng positive expression.(3)Serum gonadal hormone concentration(follicle-stimulating hormone FSH,luteinizing hormone LH and testosterone T): FSH and LH were increased in every group,but T was reduced.By making a comparison between torsion of non-necrotic testis reset group and excision group under gonadal hormone concentration(FSH,LH and T)and by making a comparison between torsion of suspected necrotic testis reset group and excision group,there was no statistical difference(P>0.05).By making a comparison on gonadal hormone concentrations(LH,FSH and T)between torsion of necrotic testis reset group and excision group,there was the statistical difference(P<0.05).Gonadal hormone co ncentrations(FSH,LH and T)were compared among torsion of non-necrotic testis reset group,excision group and torsion of necrotic testis excision group,there was no statistical significance(P>0.05).By comparing with torsion of suspected necrotic testis reset group,excision group and torsion of necrotic testis reset group,there was statistical significance on gonadal hormone concentrations(FSH,LH,T)Conclusion: 1.In the early stage of sexual maturity for rats,torsion and excisi on of non-necrotic testis had no damage for testis on uninjured side in sexual maturity.In the early stage,torsion of suspected necrotic testis might slightly damage testis of uninjured side in sexual maturity and it must be remained;2.Excision of torsion of necrotic testis in sexual materiality may protection testis in uninjured side of sexual maturity and it must be excised as soon as possible.
