Objective To study the correlation between the level of non high density lipoprotein cholesterol(non-HDL-C)and the degree of coronary artery stenosis in elderly patients with coronary heart disease.Non-HDL-C level perspective for the elderly patients with coronary heart disease to provide the basis for clinical diagnosis and treatment.Methods The clinical data of 845 patients who underwent coronary angiography examination from January 2013 and December 2014 in a 3A grade hospital were analyzed.Collect the patients' limosis serum total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),apolipoprotein A1(ApoA1),apolipoprotein B(apoB)numerical laboratory within 24 hours;then according to the above values of individual blood lipid respectively,calculate the non-HDL-C and LDL-c/HDL-c,Tc/HDL-c,apoA1/apoB numerial number.1.By coronary angiography positive(major vascular diameter stenosis greater than or equal to 50%)as the standard of diagnosis of coronary heart disease(CHD),845 patients were divided into coronary lesion group 666 cases and non coronary stenosis group 179 cases(stenosis < 50% or normal).The difference of serum lipid and(or)lipid ratio,non-HDL-C level between coronary artery stenosis group and non coronary stenosis group were analyzed.2.According to the age of patients,coronary artery stenosis group was divided into coronary artery stenosis in elderly group(age 60 years or higher)and 447 cases of coronary artery stenosis in the non elderly group(age < 60 years)in 219 cases.Difference analysis were made between coronary artery stenosis in elderly group and non elderly group of coronary artery stenosis between individual blood lipid and blood lipid ratio(or),the level of non-HDL-C.At the same time to age as independent variables,Gensini stenosis score as the dependent variables were analyzed by Pearson correlation.3.Coronary artery stenosis in the elderly group according to coronary artery stenosis Gensini integral was divided into high score group(30 points or higher)and low score group(< 30 points),of which the high score group of 133 cases,314 cases of low score group.Analyze the difference of the single blood lipid and(or)lipid ratio,non-HDL-C level between the Gensini integral high volume group and the low volume group;Analyze the correlation analysis of Gensini score in Pearson relativity;logistic regression analysis was performed to compare the blood lipid level and Gensini score of coronary artery stenosis in elderly patients,and to compare the correlation between serum lipid level and Gensini score of coronary artery stenosis.4.Coronary artery stenosis in the non elderly group according to coronary artery stenosis Gensini integral was divided into high artery stenosis Gensini integral integral group(?30)101 cases and low coronary artery stenosis Gensini integral(< 30)118 cases.Analyze the difference of Gensini and single blood lipid and(or)lipid ratio,non-HDL-C level between the Gensini integral high volume group and the low volume group in non elderly patients with coronary artery stenosis;Analyze the correlation analysis of Gensini score in Pearson relativity;logistic regression analysis was performed to compare the blood lipid level and Gensini score of coronary artery stenosis in non elderly patients,and to compare the correlation between serum lipid level and Gensini score of coronary artery stenosis.5.Analyse the LDL-C,non-HDL-C level and Gensini score of coronary artery stenosis evaluate the correlation between non-HDL-C and LDL-C level and coronary artery stenosis Gensini score through the all groups above-mentioned.Results1.To statistical analysis of 845 patients with coronary heart disease according to the coronary artery stenosis group and non coronary stenosis group,coronary lesion group of serum TC,TG,LDL-C,LDL-C/HDL-C,TC/HDL-C,non-HDL-C levels significantly higher than that of normal control group(P < 0.01 or P < 0.05),serum HDL-C?apoA1/apoB were significantly lower than normal control group(P < 0.05 or P < 0.01).The results showed that HDL-C and apoA1/apoB levels were the protective factors of coronary artery stenosis,except for HDL-C,apoA1/apoB the lipid levels above were the promoting factors of coronary stenosis.2.In the coronary artery stenosis group according to the age of the elderly patients with coronary artery stenosis and coronary artery stenosis in the elderly group were statistically analyzed.Smoking,TG levels in the two groups had no significant difference,P >0.05;gender,diabetes,the numbers of hypertension had significant difference between the two groups(p<0.05 or P < 0.01),the elderly group of patients with non-HDL-C,TC,TG,LDL-C and LDL-C/HDL-C,TC/HDL-C were significantly lower than that in the non elderly group(P < 0.01),while the level of HDL-C and apoA1/apoB were significantly higher than that in the non elderly group(p<0.05).Gensini score was regarded as dependent variables and age as independent variables,the Pearson correlation indicated=0.246(P<0.01),age and Gensini score presents positive correlation.The results showed that: except for HDL-C and apoA1/apoB,the rest of the blood lipid levels in elderly patients with coronary artery stenosis were lower than those in the non elderly group.3.According to the Gensini score,the Gensini score of coronary artery stenosis in the elderly group was divided into two groups: coronary artery stenosis Gensini integral high volume group and the low volume group.Age,gender,hypertension,diabetes,smoking,TG levels difference between Gensini high volume group and low volume group was not statistically significant p>0.05.The levels of TC,LDL-C,LDL-C/HDL-C,TC/HDL-C,and non-HDL-C were higher in the high volume group than in the low score group.The P values of the two groups were all less than 0.05,HDL-C level,and the apoA1/apoB ratio was higher in the high integration group than in the low volume group,and the P values were less than 0.01 between the two groups;The blood lipid level of the items as independent variables of coronary artery stenosis Gensini score as the dependent variable for Pearson correlation analysis found that serum non-HDL-C in coronary artery stenosis in the group(r=0.420),TC(r=0.221),LDL-C(r=0.367)and LDL-C/HDL-C(r=0.359),TC/ HDL-C(r=0.306)ratio(P < 0.01)positive correlation integral coronary artery disease and Gensini,HDL-C(r= 0.357)level,apoA1/apoB ratio(r= 0.460)(P < 0.01)integral and coronary artery lesions were negative correlated to Gensini integral.Logistic regression analysis showed that TC,LDL-C,non-HDL-C,HDL-C level and LDL-C/HDL-C TC/HDL-C ratio in the logistic regression analysis of P values are greater than 0.05,no significant correlation.ApoA1/apoB ratio was correlated with Gensini score of coronary artery stenosis in elderly patients(or=0.677,p=0.007).The results showed that TG was not associated with the severity of coronary artery stenosis in the elderly patients with coronary artery stenosis,and HDL-C and apoA1/apoB were the protective factors of coronary artery stenosis in patients with coronary artery stenosis.In addition to HDL-C,apoA1/apoB,TG levels above the level of blood lipids is an aggravating factor of coronary stenosis.4.The non elderly group were divided into two groups according to the Gensini score,the Gensini integral high volume group and the Gensini integral low volume grouping of coronary stenosis.Statistical analysis showed that: gender,hypertension,diabetes,smoking,TG levels in coronary artery stenosis Gensini high volume group and low volume group in the difference was not statistically significant p>0.05.Age,TC,LDL-C,non-HDL-C and LDL-C/HDL-C level,TC/HDL-C ratio in Gensini of coronary artery stenosis were low score group high product group.P values were less than 0.05 or 0.01.The HDL-C level and apoa1/apob ratio were lower than those of low volume group in the Gensini high score group of coronary artery stenosis,and the P values were less than 0.01.The blood lipid level of the items as independent variables of coronary artery stenosis Gensini score as the dependent variable for Pearson correlation analysis found that serum non-HDL-C in coronary artery stenosis group(r=0.515),TC(r=0.404)LDL-C(r=0.370)and ldl-c/hdl-c(r=0.454),tc/ HDL-C(r=0.401)ratio(P < 0.01)positive correlation integral coronary artery disease and Gensini,HDL-C(r=-0.252),apoa1/ apoB(r=-0.373)ratio(P < 0.01)integral and coronary artery lesions were negative correlated to gensini.Logistic regression analysis showed that TC(or7.382,p=0.011),non-HDL-C(or 2.41,p=0.013),apoa1/ apoB(or,p=0.008,)and other lipid parameters were not significantly correlated(P value >0.05).The results showed that there was no correlation between the two group in gender,hypertension,diabetes mellitus,smoking,TG.The level of HDL-C and apoa1/apob was a protective factor for coronary stenosis.In addition to HDL-C,apoa1/apob,TG levels above the level of blood lipids is an aggravating factor of coronary stenosis.5.LDL-C level and non-HDL-C level compared in all the groups mentioned.The results showed that both of them were positively correlated with the severity of coronary artery stenosis.However,the correlation coefficient of non-HDL-C between the two groups was significantly higher than that of Gensini score of coronary artery stenosis by Pearson score of LDL-C.Logistic regression analysis showed that there was no significant difference between the elderly patients with coronary artery stenosis.Logistic regression analysis of coronary artery stenosis in elderly patients with coronary artery stenosis in patients with non-HDL-C is better than the level of LDL-C.Conclusion1.The degree of coronary artery stenosis in patients with coronary artery stenosis was significantly positively correlated with the degree of non-HDL-C.2.Non-HDL-C single factor analysis was significantly correlated with the degree of coronary artery stenosis in elderly patients with coronary artery stenosis.3.Elderly group of coronary lesions,plasma apoA1/ apoB is the protect factor of coronary artery lesionsand has significant negative correlation with coronary artery lesions Gensini score(P < 0.01).4.Non conditional logistic regression analysis of the elderly patients with coronary artery stenosis and the degree of coronary stenosis were not better than apoa1/apoB.5.In non-elderly group plasma TC,apoA1/ apoB,non-HDL-C are the good indicator for coronary artery lesions.6.The correlation of non-HDL-C,LDL-C and the degree of coronary artery stenosis in each group were compared,and the non-HDL-C was better than LDL-C.
