Objective: To investigate the causes of postmenopausal uterine bleeding in 125 cases,and the relationship between age,menopause,endometrial thickness,intrauterine depth,duration of bleeding with endometrial lesions,analysing the related factors of postmenopausal uterine bleeding.Methods: We collected 125 patients treated in outpatient and inpatient department of affiliated hospital of Qinghai University due to postmenopausal uterine bleeding in January 2015 to December 2016,excluding vaginal,cervical lesions and ovarian tubal tumor-induced bleeding,inquirying their history in detail,excluding 1 year’s application of hormone drugs,checking blood levels of sex hormones,measuring their endometrial thickness with transvaginal ultrasound,using the Olympus hysteroscopy uterine cavity made by Japanese to measure endometrial depth and observe intrauterine situation,giving all patients curettage or Hysteroscopic biopsy,then sending those tissues to pathological examination.Results:Among the 125 cases of postmenopausal uterine bleeding,the first reason of bleeding is benign lesions,accounting for 77.6%(77/125),the second is malignant lesions,accounting for 22.4%(28/125).Postmenopausal uterine bleeding mostly occurred before the age of 60,accounting for 61.6%(77/125),then 53.6% of benign lesions(67/125),8.0% of malignant lesions(10/125),with the increase of age,the rate of malignancy has gradually increased tren.Postmenopausal uterine bleeding mostly occurred in menopause 1 to 5 years,accounting for 48.0%(60/125),Benign lesions accounted for the main part at this time,with prolonged menopause,the proportion of malignant tumors gradually increased trend.The incidence of malignant tumors in menopause over 10 years was as high as 15.2%(19/125).The endometrial thickness of proliferative endometrium was less than 8mm,and the endometrial thickness of endometrial carcinoma was more than 8mm,there was significant difference between the two groups(P <0.05).Compared with benign lesions,the uterine cavity depth,bleeding duration of malignant lesions,the difference was significant(P <0.05).Among the 27 cases of endometrial cancer,there was 12 cases with estrogen,accounted for 44.4%,10 cases with hypertension accounted for 32.3%,8 cases with diabetes accounted for 30.0%,overweight or obesity in 14 cases accounted for 51.9%.Conclusions:(1)Among the 125 cases of postmenopausal uterine bleeding,the reason of which was mainly benign lesion,and the malignant lesion was the second.(2)With the increase in menopause age,prolonged menopause,endometrial thickness,the rate of endometrial cancer gradually increased.(3)When the uterine volume significantly increased and bleeding duration was significantly longer,we shoud highly suspected endometrial cancer,diagnosing and treating timely;(4)High blood pressure,diabetes,obesity had a certain role in the occurrence and development of endometrial carcer,the specific still need a lot of epidemiological analysis.
