Objective: To investigate the clinical efficacy of radix notoginseng powder combined with tranexamic acid in reducing the amount of blood loss after primary unilateral total knee arthroplasty.Methods: Strict the inclusion and exclusion criteria,Fifty-two patients with moderate to severe knee osteoarthritis who were treated with unilateral TKA in Puyang Hospital of Traditional Chinese Medicine from the November 2014 to October 2016 were selected as the research object,according to random number table,the patients were divided into experimental group and control group of 26 cases as the observation object.Two groups of patients with total knee arthroplasty were performed by the Department of Orthopaedics,Puyang Hospital of Traditional Chinese Medicine.In the experimental group,the patients began to take Sanqi power with water at 1 days before the operation,once 3g with water,2 times a day to the seventh day after operation,at the same time before the close of articular cavity perfusion of tranexamic acid in surgical incision(2g tranexamic acid into0.9% Sodium Chloride Solution 50mL);The control group began to takestarchat 1 days before the operation,once 3g,2 times a day to the seventh day after operation,before the close of articular cavity perfusion of tranexamic acid in surgical incision(2G tranexamic acid into 0.9% Sodium Chloride Solution50mL).First,analyzing the general situation of the experimentalgroup and the control group(including age,sex,height,weight,Limb etc).Compared postoperative total blood loss,Intraoperative blood loss,postoperative drainage,recessive blood loss,the level of D-dimer and knee HSS score.Results: 1.The patients in the experimental group and the control group in the general data,including age,sex,height,weight,Limb etc,through the statistical analysis,no significant difference(P>0.05).2.Blood loss: The patients’ total blood loss in the experimental group was(749.48±91.61)mL;Intraoperative blood loss was(57.26 ± 14.76)mL;postoperative drainage was(215.19±49.08)mL;the hidden blood loss was(396.81±63.10)mL.In the control group,the total blood loss was(831.94±95.50)mL,Intraoperative blood loss was(60.92±15.21)mL;postoperative drainage was(220.11±34.15)mL;the hidden blood loss was(450.40±51.40)mL.Compared the total blood loss between the two groups,the experimental group was significantly less than the control group,P<0.05,it had statistical significance.Comparedthe intraoperative blood loss between the two groups,P>0.05,it had nostatistical significance.Comparedthe postoperative drainage between the two groups,P>0.05,it had nostatistical significance.Comparedthe hidden blood loss between the two groups,the experimental group was significantly less than the control group,P<0.05,it had statistical significance.3.D-dimer: Comparison of postoperative D-dimer of the two groups,There was no significant difference between the two groups(P>0.05);Comparison of D-dimer after the operation on the second day,fourth day and seventh day,the D-dimer levels of the experimental group were lower than the control group,P<0.05,it have statistical significance.In the two groups,the D-dimer on the second day after operation was significantly higher than that before the operation,and the fourth day after operation was significantly lower than the second day after operation,the difference was statistically significant(P<0.05).4.HSS score: Comparison of preoperative HSS score between the two groups,P>0.05,it was not statistically significant;Comparison of HSS score on the one month after operation between the two groups,P<0.05,The HSS score 1 months after operation was significantly different,the experimental group was better than the control group.Conclusion:Radix notoginseng powder combined intra-articular infusion of tranexamic acid has better curative effect on primary unilateral total knee arthroplasty can significantly reduce postoperative blood volume,total blood loss and hidden blood loss,intraoperative blood loss and postoperative drainage did not show significant effect,and the treatment plan obvious adverse reaction.
