Objective: To study the syndrome of the intermingled heat and cold Syndrome of acne vulgaris and establish its syndrome characteristics,to summarize the experience of Professor Liu Aimin in treating the intermingled heat and cold syndrome of acne based on syndrome differentiation,to sum up his characteristics of the clinical thinking and treating principles,methods,formulas and herbs,to further verify the unity of clinical syndrome differentiation and prescription medication.Method: By consulting the literature and consulting experts,the clinical questionnaire was designed to carry out the clinical epidemiological investigation,and build a data base of the collected clinical medical records of Professor Liu Aimin in treating the intermingled heat and cold syndrome of acne based on syndrome differentiation.Then utilize SPSS19.0 statistical software for frequency analysis,cluster analysis and principal component analysis to analyze the traditional Chinese medicine(TCM)syndrome and clinical characteristics of the intermingled heat and cold syndrome of acne,count the frequency and proportion of his clinical prescription and extract the basic prescription.Result:(1)The intermingled heat and cold Syndrome of acne vulgaris is divided into two syndromes: the upper heat and lower cold syndrome and the liver-heat and spleen-cold syndrome.(2)70 cases are collected in clinic: 50 cases of the upper heat and lower cold syndrome and 20 cases of liver-heat and spleen-cold syndrome.The first syndrome includes 6 male(12%)and 44 female(88%),the sex ratio is 1:7.33.The ages range from 11 years old to 47 years old and the mean age is 26.76±6.90.The second syndrome includes 7 male(35%)and 13 female(65%),the sex ratio is 1:1.86.The ages range from 17 years old to 35 years old and the mean age is 24.65±4.21.29 cases(41.43%)were between 10 to 24;40 cases(57.14%)were between 25 to 39;1 case(1.43)was above 40 years old.(3)The TCM symptoms of the upper heat and lower cold syndrome of acne are loose stool,abdomen pain with diarrhea caused by cold dish,pale tongue,be susceptible to cold,weak pulse,chilly sensation and the cold limbs,lusterless face,tongue red at the tip and margins,greasy fur,thready pulse,preferring hot dish,thready pulse,sticky stool,white tongue fur.The clinical characters are light red or red of the skin lesions,skin lesions on forehead,perioral and perinasal,history of cold medicine,acne,pimples and pustules.(4)The TCM symptoms of the liver-heat and spleen-cold syndrome of acne are being irritable,chilly sensation and the cold limbs,abdomen pain with diarrhea caused by cold dish,stringy pulse,perturbed,thirst,dry or sticky stool,pale tongue,tongue red at the tip and margins,thready pulse,sallow complexion,dysmenorrheal,white tongue and greasy fur.The clinical characters are skin lesions exacerbation in premenstruum,skin lesions in cheeks and jaw,aggravation under pressure,oily skin,red skin lesions,acne,pimples,tubercles and pustules,history of cold medicine.(5)112 prescriptions are collected among the 70 cases,78 prescriptions of the upper heat and lower cold syndrome and 34 prescriptions of the liver-heat and spleen-cold syndrome.The basic prescription of the first syndrome consists of Scutellaria Root,Dried Ginger,Forsythia,Dandelion,Goldthread,Dried Tangerine Peel,Licorice,Schizonepeta,and Pinellia Ternata.The basic prescription of the other syndrome consists of Bupleurum Root,Dried Ginger,Forsythia,Dried Tangerine Peel,Scutellaria Root,Dandelion,Licorice,Atractylodes,Guizhi and Fructus Gardeniae.Conclusion:(1)The intermingled heat and cold Syndrome of acne vulgaris is divided into two syndromes: the upper heat and lower cold syndrome and the liver-heat and spleen-cold syndrome.(2)The specific TCM symptoms of the upper heat and lower cold syndrome of acne are loose stool,abdomen pain with diarrhea caused by cold dish,pale tongue,be susceptible to cold and weak pulse;the main symptoms are chilly sensation and the cold limbs,lusterless face,tongue red at the tip and margins,greasy fur and thready pulse;the minor symptoms are preferring hot dish,thready pulse,sticky stool,white tongue fur.The specific symptoms of its clinical characters are light red or red of the skin lesions,skin lesions on forehead and history of cold medicine;the main symptoms are acne and pimples;the minor symptoms are pustules.(3)The specific TCM symptoms of the liver-heat and spleen-cold syndrome of acne are beingirritable,chilly sensation and the cold limbs,abdomen pain with diarrhea caused by cold dish and stringy pulse;the main symptoms are perturbed,thirst,dry or sticky stool,pale tongue,tongue red at the tip and margins and thready pulse;the minor symptoms are sallow complexion,dysmenorrheal,white tongue and greasy fur.The specific symptoms of its clinical characters are skin lesions exacerbation in premenstruum,skin lesions in cheeks and aggravation under pressure;the main symptoms are oily skin,red skin lesions,acne,pimples,tubercles and pustules;the minor symptoms are skin lesions in jaw and history of cold medicine.(4)The prescription of the upper heat and lower cold syndrome is based on Banxia Xiexin Decoction,consists of Scutellaria Root,Dried Ginger,Forsythia,Dandelion,Goldthread,Dried Tangerine Peel,Licorice,Schizonepeta,and Pinellia Ternata.The prescription of the liver-heat and spleen-cold syndrome is based on Chaihu Guizhi Ganjiang Decoction,consists of Bupleurum Root,Dried Ginger,Forsythia,Dried Tangerine Peel,Scutellaria Root,Dandelion,Licorice,Atractylodes,Guizhi and Fructus Gardeniae.
