The Clinical Research On The Effect Of Adolescent Polycystic Ovary Syndrome With Insulin Resistance By The Decoction Of Bushen Ditan Tang

Objective:take "renal deficiency and dampness" as the main pathogenesis,and explore the mechanism of Bushen Ditan Decoction for treating adolescent PCOS with insulin resistance from the perspective of clinical application and theory.To analyses the clinical effect of Bushen Ditan Decoction in treating puberty PCOS combined with insulin resistance.method:The study divides 60 cases of adolescent PCOS patients with insulin resistance who met the inclusion criteria into two groups:the treatment group(30 cases of Bushen Ditan Decoction)and the control group(30 cases of metformin group),and they were continuously treated for 3 months.Observe traditional Chinese medicine symptoms,hair,acne,body mass index(BMI),Waist-to-hip ratio(WHR),Basic body temperature(BBT),Ovarian morphology,etc.before and after treatment;analyze the changes of hormone levels(FSH,LH,T,SHBG),fasting insulin(FINS),fasting plasma glucose(FPG),and calculate the homeostasis model assessment insulin resistance index(HOMA-IR),free androgen index(FAI).Results:(1)Comprehensive efficacy:The total effective rate of the treatment group was 86.7%,and 73.3%in the control group.The treatment group was superior to the control group(P<0.05).(2)Sex hormone levels:After treatment,the levels of serum LH,LH/FSH,and FSH in the control group decreased(p<0.05).The level of serum FSH in the treatment group was no difference compared with before treatment(P>0.05).There was no difference between two groups(p>0.05).The levels of SHBG in the two groups increased after treatment,and the levels of T and FAI decreased(p<0.05).There was no difference between two groups(p>0.05).(3)Insulin resistance:HOMA-IR and FINS in two groups decreased after treatment(p<0.05),and there was no difference between the two groups(p>0.05).No significant changes in the FPG of both groups before and after treatment(p>0.05)(4)BBT:After treatment,both groups of BBT biphasic significantly recovered(P<0.01),and no significant difference exists between the two groups(p>0.05).(5)The scores of related symptoms:After treatment,the traditional Chinese medicine symptom scores of the two groups were decreased(P<0.05).The treatment group was superior to the control group(p<0.05).There was no difference in hirsutism scores,acne scores,and ovarian integral scores between the two groups(p>0.05).Within the group,acne scores and ovarian scores were decreased in the treatment group(P<0.05).(6)BMI and WHR:BMI and WHR of two group were lower than those before medication(P<0.05),and there was no difference between two groups(p>0.05).Conclusion:Bushen Ditan Decoction can improve ovulation status,sex hormone levels and insulin resistance in adolescent PCOS patients with IR puberty.Compared with metformin in the control group,there is no significant difference.The Comprehensive efficacy and improvemen of traditional Chinese medicine symptoms in Bushen Ditan Decoction is better than that of the control group,and no toxic side effects.Bushen Ditan Decoction provides an effective treatment of traditional Chinese medicine for the adolescent PCOS patients with IR,which is worth further clinical application.
