Objective To investigate the effects of Hemorrhagic Shock on Pharmacodynamic of Propofol in the plateau area.Methods 24 Landrace were randomly divided into 4 groups,with 6 Landracein each group: plain group(group C),plain hemorrhagic shock group(group CS),plateau group(group G),and plateau hemorrhagic shock group(group GS).The Landrac were transported from Lanzhou(altitude of 1503 m)to Yushu(altitude of 4120 m)within 24 h in Plateau group and plateau hemorrhagic shock group,Anesthesia was induced with sevoflurane,ach Landrace was intubated.Landrace in group GS were established volume controlled hemorrhagic shock by removing blood from right femoral artery.Propofol(150μg/kg·min))was infused intravenously over 10 min in group G and group GS.2ml arterial blood collected at 2,4,6,8,10,11,12,13,14,15,17.5,20,25,30,45,60,90,120,and 180 min after the start of the infusion,.and Propofol concentrations were measured.Narcotrend values were recorded at each time point,Animals in group C and group CS were operated in animal experimental section of Lanzhou General Hospital of Lanzhou military area command.The experimental process of group C was the same as the G group,and the CS group was the same as the GS group.Results Electroencephalogram inhibition time in group C was longer that of group G(P<0.05):Electroencephalogram inhibition time in group GS was shorter thoes of group Gand group CS(P<0.05).Narcotrend values in group C was higher that of group G at Each time point after propofol infusion(P<0.05),Narcotrend values in group GS was lower thoes of group Gand group CS at Each time point after propofol infusion(P<0.05)Propofol EC50 in group G was significantly lower than group C(P<0.05).Propofol EC50 in group G was significantly higher than thoes group G and group CS(P<0.05).Conclusion The dosage of propofol needed to maintain the same equivalent depth of anesthesia in plateau volume controlled hemorrhagic shock was significantly lower than that of plain,plateau and plain volume controlled hemorrhagic shock.The EC50 of propofol was 9.0±1.1μg/ml in plain group,4.6±0.9μg/ml in plain hemorrhagic shock group,8.1±0.6μg/ml in plateau group and 3.1±0.96μg/ml mu g/ml in plateau hemorrhagic shock group.It illustrateed the end-organsensitivity of the Landrace to propofol increased,the effect increased,the dosage decreased and the action time prolonged.
