Objective: To understand the health literacy level of residents in different genders,age groups and educational levels by contrasting and analyzing the survey data of urban and rural residents in Hebei Province in 2012 and 2016.From the overall level,three aspects and six kinds of health issues of the health literacy,learn the health literacy status about the urban and rural residents in Hebei Province and discover weaknesses of residents' health literacy.In future,it will provide data support for carrying out targeted health education and health promotion work,and provide scientific basis for making appropriate strategies and measures.Methods: A multi-stage stratified cluster random sampling method was used to investigate the permanent residents,who were aged 15 to 69 years in Hebei province.In 2012 and 2016,the National Health Care Quality Monitoring Questionnaire was used in the survey,which was compiled by the National Health Commission.The survey was conducted by the investigators to conduct a face-to-face survey and strict quality control.According to the criteria of health literacy,the questionnaire was used to calculate the overall level of health literacy,including three aspects of health literacy: the basic health concept and knowledge,healthy lifestyle and behaviors,basic health skills;scientific attitude towards health,infectious disease prevention,chronic disease prevention,safety and first aid,basic medical care,health information in six types of health issues of health literacy.Data was entered by using Epidata3.1 software(double entry verification),and statistical analysis was using SPSS 21.0 statistical software.P<0.05 indicated the difference was statistically significant.Data analysis methods were compared using descriptive analysis,chi-square test and so on.Results:1 In 2012 and 2016,a total of 3217 people and 4075 people were investigated in Hebei Province.The overall level of adjusted health literacy(AHL)among the Hebei residents were 9.50 percent in 2012 and 7.40 percent in 2016.2 In 2012,health literacy level of three aspects in Hebei province were:basic health concept and knowledge 14.63%,healthy lifestyle and behaviors11.94%,basic health skills 8.94%.The health literacy rate of six health issues as safety and first aid,scientific attitude towards health,health information,infectious disease prevention,chronic disease prevention,and basic medical care were 38.34%,29.12%,17.35%,12.11%,10.75%,and 9.15%.3 In 2016,health literacy level of three aspects in Hebei province were:basic health skills 12.10%,basic health concept and knowledge 11.80%,healthy lifestyle and behaviors 8.00%.The health literacy rate of six health issues as safety and first aid,scientific attitude towards health,infectious disease prevention,basic medical care,health information and chronic disease prevention were 40.20%,25.70%,20.00%,15.50%,8.70%,and 3.50%.4 Compared with 2012,the literacy level of basic health skills increased,the literacy levels of basic health concept and knowledge,healthy lifestyle and behaviors decreased(P<0.05).In 2016,the literacy level of safety and first aid among the urban and rural residents remained unchanged(P>0.05),the literacy levels of basic medical care,infectious disease prevention increased,and the literacy levels of health information,scientific attitude towards health and chronic disease prevention decreased(P<0.05).Conclusion:1 In 2012 and 2016,the health literacy overall level is lower.2 Compared with 2012,the level of basic health concept and knowledge and the level of healthy lifestyle and behaviors declined,and it's difficult to transfer health knowledge into action.3 The level of health information acquirement and understanding is not high and the level of chronic disease prevention is very low,that can not meet the need for maintaining and promoting health.4 Compared with 2012,the health literacy overall level declined in 2016 mainly due to the distribution of urban and rural areas,education level and age factors,suggesting that we should focus on people in rural area,high age group and lower level of education.
