Clinical Observation On Treatment Of Primary Glomerular Disease Proteinuria With Buqi Jianpi Yishen Hufeng Method

Background:Proteinuria is not only a common clinical symptom of kidney disease,but also an important factor to aggravate renal injury.The increase of protein filtration in circulation can also lead to tubular epithelial cell injury and tubulointerstitial fibrosis.In the observation of the curative effect of Ramipril on nephropathy,it was found that proteinuria was the only indicator associated with the decrease of glomerular filtration rate in patients with non-diabetic CKD.Therefore,clinical control of proteinuria is an important target of treatment.At present,steroids,immunosuppressants,Ras blockers and anticoagulants etc are routine treatments,although these traditional treatments for nephropathy have been proved to be effective in reducing proteinuria and protecting renal function,but the use of steroids and immunosuppressants has certain indications,mainly used in patients with nephropathy level proteinuria,it is not suitable for all primary glomerulonephritis patients.There are easy to relapse after withdrawal,steroids dependence,steroids resistance and other side effects.Protein belongs to the category of "fine substance" in traditional Chinese medicin e,and the pathogenesis of proteinuria is discussed differently.Most doctors think that proteinuria is based on deficiency of spleen and kidney,and exogenous evil of six 1 ewdness is the standard,from the angle of wind,wet and blood stasis,the method of promoting the lung,invigorating the spleen and regulating the kidney.According to t he clinical experience,Professor Wang Baokui suggested that the importantpathogenic factors in the occurrence and development of proteinuria in primary glomerular diseas es was wind evil,wind catharsis,lung health damage,wind evil guest in the muscle surface,along the meridian,direct invasion of the kidney,induced proteinuria.,or the water and dampness stagnation injures the spleen and kidney,formation of proteinuria,self-prescription of traditional Chinese medicine for deficiencyof spleen and kidney,wind evil into the kidney type proteinuria has a good clinicalefficacy.Astragalus,Ach yranthes bidentata,Chuanshanlong and other traditional Chinese medicine prescription has the function of invigorating qi and invigorating spleen,tonifying the kidney and di spelling wind.By observing the therapeutic effect of traditional Chinese medicine on proteinuria of primary glomerular disease,the clinical effect of traditional Chinese me dicine compound was discussed,which provided a new reference method for the treat ment of primary glomerular disease.Objective:To observe the therapeutic effect of self-made prescriptions on proteinuria of primary glomerular diseases with deficiency of spleen and kidney and pathogenic wind into kidney,to explore the thinking of syndrome differentiation and prescription,and to improve the clinical remission rate of patients with primary glomerular diseases.Methods:To collect the patients with proteinuria of primary glomerular disease according to the standard of inclusion,combined with traditional Chinese medicine on the basis of the original western medicine treatment,to compare the changes of laboratory indexes and TCM syndrome before and after treatment,and to use SPSS20.0 software to carry on statistical analysis.Results:1.after 12 weeks of treatment,the urinary protein excretion rate decreased,the dif ference of rank test was statistically significant,(P<0.05).2.Evaluation of therapeutic ef fect of proteinuria:the total efficiency of 94%,4 cases of clinical control,25 cases markedly effective,4 cases invalid.3.Evaluation of curative effect of TCM Syndrome:the total effective rate was 89%,there were 0 cases of clinical cure,5 cases of rema rkable effect,26 cases of effective and 4 cases of failure.4.Other laboratory indexes:s erum albumin increased,serum creatinine and total cholesterol decreased,there was sig nificant difference before and after treatment,(P<0.05).Conclusions:1.The basic pathogenesis of proteinuria of primary glomerular disease is deficiency of spleen and kidney qi,wind evil enters the kidney.2.Chinese herbal compound can effectively reduce the urine protein of patients with primary glomerular disease caused by deficiency of spleen and kidney and wind evil.3.This prescription can increase the serum albumin level and reduce the total cholesterol level in patients with primary glomerular disease proteinuria.4.Traditional Chinese medicine compound can improve many kinds of clinical discomfort.
