| Objective:To explore the feasibility and efficacy of extended nursing based on WeChat platform for patients with total hip replacement in postoperative rehabilitation period.Methods:A total of 60 cases of patients with total hip replacement at the hospital in Changchun,were randomly divided into the observation group and the control group(n=30).The control group received general guidance and education at discharge,including dietary guidance,proper medication,daily activity,improper training,matters needing attention,prevention of complication and reexamination time.The observation group was given six-month extended nursing based on WeChat platform after discharge.In particular,WeChat was used to send and receive texts,voices,pictures and videos to educate and guide patients outside the hospital.According to WeChat time and guidance content,the extended nursing was consist of three parts:(1)WeChat time for one-on-one treatment: guide patients to use antithrombotic drugs(such as rivaroxaban)and analgesics(like coddalon and celebrex)correctly.At the same time,advise patients to have a reasonable diet in the early(1~2 weeks),middle(2~4 weeks)and later(5 weeks)postoperative periods.To evaluate the functional recovery of hip jointand identify the incidence of complications.Meanwhile,systematic and scientific rehabilitation training and nursing guidance would be provided for patients and their families.If necessary,don’t mind contacting your doctor.(2)WeChat time for wardmate groups: The typical cases were analyzed in WeChat group;In addition,The changes of the disease,causes of complications and related solutions could be discussion among patients;And patients at early postoperative could communicate with other patients with good functional recovery.(3)WeChat time for Fixed Investigation: The recovery status of patients were evaluated in 3d,7d,14 d,21d and 28 d after discharge through WeChat,and then the recovery status were assessed once a month until the 6th month.Harris hip score(including at the time of discharge,1,3 and 6 months after discharge),postoperative adverse events and nursing satisfaction were compared between the two groups.Results:Sex composition,mean age and disease distribution had no significant difference between the two groups(P>0.05).Harris hip score at the time of discharge,1,3 and 6 months after discharge were(43.48 ± 2.80),(54.31 ± 3.40),(62.62 ± 3.81),(70.99 ± 5.37)in the control group respectivey,and(43.96 ± 2.18),(55.43 ± 3.35),(73.19 ± 4.99),(83.82 ± 6.68)in the observation group respectivey.Harris hip score at the time of discharge and 1month after discharge had no significant difference between the two groups,but a significant difference at 3 and 6 months after discharge between two groups were viewed.The incidence of adverse events in the control group is10.0%(dislocation 2 cases,infection 1 case),which is higher than that of observation group(P<0.05).The nursing satisfaction in the observation group is 96.67%,which is significantly higher than that in control group(76.67%),P<0.05.Conclusion:The extended nursing based on WeChat platform is practical and worthy for clinical use,which can effectively educate and guide patients with total hip replacement in postoperative rehabilitation period.As results,improving the recovery of limb function,the incidence of adverse events could be obviously reduced,and the patients’ satisfaction for the quality of care would be increased. |
