BackgroundTotal Knee Arthroplasty(TKA)is the best method for the treatment of advanced knee osteoarthritis,but it is still a challenge to maintain an effective treatment effect,and it is urgent for medical staff to provide professional,long-term and timely continuity nursing services to improve the sustainability of postoperative treatment and the effectiveness of rehabilitation exercises.However,due to the current reality of insufficient medical resources and uneven distribution,it cannot effectively meet the implementation of continuing nursing services for patients outside the hospital.Due to its simplicity,convenience,efficiency,and mobility,the We Chat platform has been widely used as a means of health communication in continuity care services for discharged patients.ObjectiveBy investigating the continuous care needs of patients after TKA and analyzing the influencing factors,the continuous care intervention program of We Chat platform was constructed,and the application research effect of the intervention program on patients after TKA was evaluated,aiming to provide a scientific basis for the formulation of continuous care model for patients after TKA.Methods1.The convenience sampling method was used to investigate 356 patients after TKA from 5 tertiary general hospitals in northern Henan Province.The questionnaire of general information,the scale of continued care needs of middle-aged and elderly patients undergoing total knee replacement,the scale of hospital for special surgery knee score,the mos 36-item short form health survey,pain catastrophizing scale were used to investigate.SPSS20.0 software was used for statistical description,independent sample u test,one-way analysis of variance,Pearson correlation analysis and multiple linear regression analysis.2.Combining the literature review and the results of the previous needs,a preliminary construction of a continuous care plan for the We Chat platform was established,and expert group meetings were used to discuss and include professionals in the fields of medical care,nursing,and information technology to revise and establish the plan.3.The convenience sampling method was used to select 80 patients after TKA who were hospitalized in a tertiary general hospital in Xinxiang City from January 2020 to December 2020 as the research object,and 40 patients who were hospitalized from January2020 to June 2020 were included in the control group,40 patients hospitalized from July2020 to December 2020 were included in the intervention group.Patients in both groups received routine nursing,while patients in the intervention group received continuous nursing intervention on the basis of We Chat platform.In the pre-intervention,1 month and3 months after intervention,questionnaires were used to investigate the general information,knee joint function,quality of life and pain catastrophic level of the patients.After 3 months of intervention,the We Chat intervention scale was used to investigate the application effect of the We Chat platform in the intervention group.SPSS20.0 software was used for statistical description,independent sample t test,?2 test and repeated measures analysis of variance.Results1.The total score of patients’ needs for continuous care after TKA was(110.47±7.08)points,which was at a medium level.68.5% of patients hoped to provide continuous care services through the network communication platform.The patient’s quality of life,age,and knee joint function were the main influencing factors for the continuing care needs of patients after TKA(P<0.05).2.After a round of expert group meetings,nine professionals in fields of medical,nursing,and information technology discussed the continuation care plan of the We Chat platform for patients after TKA,and finally revised the intervention plan.3.A total of 80 patients were enrolled,40 in the control group and 40 in the intervention group.During the course of the study,neither group was lost to follow-up or dropped out.There were no significant difference in general information between the two groups of patients(P>0.05).Before the intervention,there were no statistically significant difference in the indicators between the two groups of patients(P>0.05).After the intervention,the knee joint function,pain catastrophe and quality of life scores of the intervention group were better than those of the control group at 1 month and 3 months,and the differences were statistically significant(P<0.05).The intergroup effect,time effect and interaction effect of knee joint function,pain catastrophic level and quality of life of the two groups of patients at different time points were statistically significant(P<0.05).Patients in the intervention group had a high degree of acceptance and satisfaction with the continuation of the We Chat platform.Conclusions1.Continuing care needs of patients after TKA were at a medium level,and the content showed a trend of diversification,and the ways of communication were mostly on the network platform.Age,quality of life and knee function were the influencing factors of continuing care needs.2.The continual care plan based on the We Chat platform can effectively promote the recovery of knee joint function of patients after TKA,improve the level of pain catastrophe of patients,and improve the quality of life of patients.
