| Objective: To investigate the situation of self-efficacy in patients with enterostomy in Xinjiang and to analyze the influence of demographic variables,stoma-related conditions,adaptation,social support,anxiety / depression on self-efficacy of patients with enterostomy,self-efficacy of the factors that affect the clinical nursing staff to improve the level of self-efficacy of patients with enterostomy nursing intervention provide a reference.Methods:1.Cross-sectional investigation: Using the method of typical survey,from December 2016 to August 2017 in the five tertiary hospitals in Xinjiang,the demographic and disease-related questionnaire,Ostomy Adjustment Inventory-Chinese Version,Stoma Self-efficacy Scale,Social Support Revalued Scale and Hospital Anxiety / Depression Scale were used for questionnaire survey of 220 patients with enterostomy.2.Statistical methods: using SPSS 22.0 statistical software for data analysis,description of statistical analysis using frequency,mean,standard deviation.Univariate analysis used t-test,analysis of variance and non-parametric test.Describe the correlation between the two scales using Pearson correlation analysis.The single factor and the correlation analysis statistically significant variables were analyzed by multiple stepwise regression.Results:1.The total score of self-efficacy in patients with stoma was(67.72±14.23),which was moderately low;the score of care efficacy was(35.46±7.13);the score of social efficacy was(18.27±5.29).In this study,91(41.4%)patients were at low efficacy level,125(56.8%)were at moderate efficacy level,and only 4(1.8%)patients were at high efficacy level.2.The total score of patients with intestinal stoma was(48.91±10.31),which was moderate level.The total social support score was(37.67±5.20),at a moderate level.The scores of anxiety and depression were(3.63±3.13)and(4.43±3.29)respectively.3.Univariate analysis showed that age,body mass index,personality,marital status,postoperative time,the main source of cost of purchasing stoma products,monthly cost of purchasing stoma products,degree of self-care,complications,coping attitude and degree of communication with doctors and nurses were the influencing factor of self-efficacy(P<0.05).Correlation analyses revealed a statistically significant positive correlation between acceptance,positive attitude to life,social support and self-efficacy in patients with enterostomy(P<0.01)and a significant negative correlation between constant worry,anxiety/ depression and self-efficacy in patients with enterostomy(P <0.01).4.The results of multiple stepwise regression showed that constant worry,depression,degree of self-care,postoperative time,subjective support,age,positive attitude towards life,anxiety entered into the regression model,Beta coefficients were-0.238,-0.233,0.196,0.242,0.123,-0.116,0.143,-0.082,8 variables explain 80.5% of total variation of self-efficacy.Conclusion:1.The self-efficacy of patients with stoma was at medium/low level,which needs to be improved urgently.2.The postoperative time of stoma had the greatest influence on patients’ self-efficacy;the anxiety of the least affected,depression has a greater impact on patients.The longer the postoperative stoma,the better the self-care degree,the higher the self-efficacy of patients with enterostomy who have a positive attitude to life;the lower self-efficacy of patients with higher age,persistent worry,anxiety and depression should be given sufficient attention to.3.To improve the patient’s enthusiasm for life,to alleviate the anxiety and depression,encouraging patients to self-care stoma as soon as possible,to guide relatives to give social support for patients,can improve the level of self-efficacy of patients.In clinical nursing and extended care,we should explore intervention methods around these aspects to increase the patient’s level of self-efficacy,and ultimately improve the quality of life of patients. |
