1 Objective1.1 To Analysis the characteristics of 155 cases of clinical medical records of LC,and to evaluate the clinical efficacy of acupoint application.1.2 To observe the effects of acupoint application on gastrointestinal function and postoperative anxiety and depression after LC,evaluate clinical curative effect of Acupoint Application relatively objectively in this study.2 Methods2.1 With the method of retrospective analysis,we analyzed 155 cases who accepted LC surgery from 2016.1 to 2017.1,the investigation content included general situation, the postoperative clinical symptoms, use of Chinese medicine external treatment and so on,and we used Fisher exact probability statistics to analyze the clinical curative effect of acupoint application.2.2 80 patients were selected as subjects from the Department of general surgery of Mianyang Hospital Affiliated to Chengdu University of Traditional Chinese Medicine, who underwent LC during the 2017.6-2018.2.Through clinical research methods, all subjects were randomly divided into the experimental group(acupoint plaster therapy)and the control group(placebo treatment),and the two groups were treated with both sides of Zusanli and Shangjuxu. Two groups of subjects were treated immediately after returning to the ward with application for 4 hours. From the second day after the operation, the application was applied at 8 a.m for 4 hours, until the first defecation.Recorded and compared the recovery time of intestinal gurgling, postoperative first exhaust and defecation time, postoperative nausea and vomiting, abdominal distention, and postoperative wound pain etc.at 6h,24 h,48h and 72 h after operation;Analyzed and compared the anxiety and depression in the first day of the surgery and the 48 h after surgery with Self-rating Anxiety Scale(SAS)and self-rating depression self-rating Scale(SDS).3 Results3.1 In the 155 cases of retrospective analysis of the medical records,a total of 138 patients were treated with acupoint application, moxibustion, electroacupuncture,Chinese medicine hot bag,ear point pressure bean, and 17 cases were not treated by any method.Acupoint application was used mostly in external treatment of Traditional Chinese Medicine after LC.Acupoint application compared respectively with the method of moxibustion, herb hot package method, the ear point pressure and untreated, first exhaust and defecation time of Acupoint application was most obvious, the difference had significant statistically(P<0.01),there were difference in the symptoms of postoperative nausea, vomiting and abdominal distention(P<0.05);there was no significant difference between acupoint application and electroacupuncture(P>0.05).3.2 68 subjects finished this clinical study.(1)Comparison of gastrointestinal function:the postoperative bowel sounds, first exhaust defecation time of the test subjects was significantly earlier than that of control group(P<0.01);there were difference of nausea at 6h,24 h,48h and 72 h after operation between the two groups(P<0.05);After 6h,there was difference between the two groups of vomiting(P< 0.05);After the operation, the degree of abdominal distention of 6h and 24 h was different in the two groups(P<0.05);there were differences in pain scores between two groups after 6h,24 h,48h and 72 h.(2)Comparison of anxiety and depression:there were significant differences in scores of SAS and SDS at 1 day before surgery and admission?48h after surgery and 1 day before surgery in two groups(P<0.01);there was no difference in scores of SAS and SDS at admission,1 days before operation and 48 h after operation between two groups(P>0.05).4 Conclusion4.1 There were many methods for the treatment of TCM in case of LC,among which Acupoint application had the characteristics of higher frequency of use and better effect on improving gastrointestinal symptoms, however, there was a lack of standardized clinical evaluation results.4.2 The application of the randomized controlled clinical study basically confirmed that Acupoint application had the effect of promoting the recovery of gastrointestinal function and reduce postoperative wound pain after LC,but the impact on anxiety and depression were not apparent.
