SVF-gel Assisted Autologous Fat Grafting In Facial Rejuvenation

ObjectiveTo investigate the effect of SVF-Gel assisted autologous fat grafting in Facial Rejuvenation.Methods1、Design:Design personalized fat grafting range and mark according to the different facial contours of the surgeon in their face.2、Anesthesia: Use 2% lidocaine with 0.1% epinephrine added infiltration anesthesia into the needle point.Anesthesia in the source area uses tumescent technique.Area for tumescent anesthesia needle point and the area into the needle point using the same infiltration anesthesia.3、Preparation of fat granular and SVF-Gel:Select the leg or hypogastrium as the fat granular source area.Use a 20 ml syringe to connect the liposuction needle after 5-10 min to create a manual negative pressure and then a repeated multi-layered,multi-tunnel fat suction operation is performed in the subcutaneous fat layer as fan-shaped.After taking the desired amount of fat suspension and standing for about 10 minutes,it is seen that the fat becomes stratified and then exclude the lower bloody swelling fluid.The resting adipose tissue was aliquoted in a 10 ml sterile centrifuge tube and centrifuged(1200 g * 3 min)and then collect the uppermost droplets and remove the lowermost layer of liquid to obtain Coleman fat,which is granular fat.Take half of the granule fat in two 10 ml syringes.Connect the fat emulsifier to fully emulsify the fat and centrifuge for the second time(2000g * 3min),remove the top oil droplets and the bottom of the liquid,showing the middle layer was jelly mixture that SVF-Gel.All of the prepared SVF-Gel and the other half of the fat particles were thoroughly mixed and prepared for injection and transplantation.4、Injection method: Select the face of the needle point from the transplant area edge of about 1cm relatively hidden place,in the needle point for local anesthesia and then according to the transplant area selection of different diameter of the transplanting needle from the puncture point into the subcutaneous and to the depression by the farthest end to the area slightly bulging or full filling.The total amount of injection can be greater than the required injection.Injection should be careful to try to fill the fat evenly throughout the three-dimensional space in the area,and the natural convergence with the surrounding.ResultsBetween January 2017 to March 2018.Application of SVF-Gel assisted autologous fat grafting in facial rejuvenation in 48 cases,which also fill the frontotemporal,metatarsal,periorbital,cheek,nasal back,nasolabial fold,chin or filling three or more parts in 28 cases,Frontotemporal in 4 cases,filled cheek in 5 cases,filled with nasolabial fold in 4 cases,3 cases were filled with chin,and 4 cases were filled with nasal.Among them,only 45 cases of satisfactory results after once operation were performed.The second operation was performed in 3 cases because of the satisfactory facial rejuvenation effect was achieved after transplantation.No cases of infection,necrosis,induration,fat embolism and other serious complications in this group of patients.ConclusionThe application of SVF-GEL-assisted fat transplantation in facial rejuvenation is effective,and most of them can achieve satisfactory results through one operation.
