| Background:Cervical squamous epithelial lesions and cervical cancer are common clinical diseases,and the pathogenesis is very complex.It is generally considered to be related to HPV viral infection,sexual behavior and number of children and other biological factors,such as smoking.There is no specific clinical manifestation at early stage of the disease.With the continuous development of the disease,more manifestations such as vaginal bleeding because of sexual life or other factors such as a gynecological examination,vaginal discharge,severe cases may be associated with bowel defecation,abnormal urination and even difficulty,lower extremity swelling,ureteral obstruction,and so on,affecting the health and life of patients.Epidemiology and biology confirmed that HPV infection is a related factor causing cervical cancer and precancerous lesions.More than 90%of cervical cancer is associated with HPV infection,but HPV infection has strong regionality,HPV infection in different countries and regions.There are obvious differences in rate and type.Therefore,strengthening the research of correlation between cervical lesions and HPV infection in Linyi area can provides a basis for intervention in clinical treatment.Objective:To investigate the correlation between cervical squamous epithelial lesions and cervical cancer and HPV infection in Linyi area,which to provide basis and reference for clinical diagnosis and treatment.Methods:The objects of study were 434 patients with cervical squamous epithelial lesions or cervical cancer in two years(February 2016-February 2018).They were identified by associated examination of cervical TCT and HPV type,and were diagnosed by colposcopy.HPV typing was performed by gene chip method to meditate HPV infection status;for patients with positive HPV test,cervical cytology or colposcopy,SPSS Pearson correlation analysis software was used to analyze the correlation peak between cervical lesions and HPV infection.Results:(1)(1)Of the 434 patients were eventually diagnosed in Linyi area,including 160 cases of low-grade cervical lesions,224 cases of high-grade cervical lesions and 50 cases of cervical cancer.All patients were given HPV specimens of cervical secretion before operation,and the patient’s HPV classification was positive in 346 cases,including 112cases of low-grade cervical intraepithelial neoplasia,188cases of high-grade cervical intraepithelial neoplasia,and 46 cases of cervical cancer.The infection rates accounted for 70.00%,83.93%,and 92.00%.respectively.(2)HPV typing in low-grade cervical lesions was mainly concentrated in HPV16(30 people)and HPV58(26 people),respectively.Accounted for:30.36%and 23.21%;HPV typing in patients with high-grade cervical lesions was mainly concentrated in HPV16(102 people)and HPV33(42 people)accounting for 54.26%and 22.34%,respectively;cervical cancer HPV typing was mainly concentrated in HPV16(30 people)and HPV18(6 people),accounting for 65.22%and 13.04%;(3)Cervical cancer patients mostly mainly single infection,accounting for about 86.96%,are higher than LSIL and HSIL(P<0.05);LSIL and HSIL dual infections,triple infections and multiple infection rates were higher than cervical cancer(P<0.05)(4)The results of single factor and multi-factor Logistic analysis showed that the positive rate of HPV in patients with cervical lesions was statistically significant with the age of first sex,the number of pregnancies,the use of condom contraception and the number of sexual partners(P<0.05).(5)SPSS Pearson the results of the analysis software showed that the incidence of cervical cancer in Linyi area was positively correlated with HPV16 and HPV18(P<0.05).The incidence of cervical squamous epithelial lesions in Linyi area was not only related to HPV16 and HPV18,but also had a close relationship with HPV58 and HPV33.Conclusion:(1)HPV infection in Linyi area is mainly HPV16,HPV18,HPV58 and HPV33.It is suggested that HPV16 and HPV18 are the main subtypes of cervical lesions,which must be closely observed。However,HPV58 and HPV33 can cause cervical intraepithelial neoplasia and need to be closely observed,which can intervention and treatment to avoid progression to cervical cancer timely.(2)The different levels of cervical cancer in high-risk type HPV infection exists obvious difference,high-risk type HPV infection is the main cause of cervical cancer occur,for this area provides the theory basis for early diagnosis and prevention of cervical cancer. |
