Objective:In this study,We measured the levels of serum CA125 and BNP,and evaluated cardiac function with color Doppler echocardiography of patients with coronary heart disease complicated by chronic heart failure and healthy controls,to explore the clinical significance of levels of serum CA125 with coronary heart disease complicated by chronic heart failure.Method:We studied a consecutive cohort of 160 patients admitted for CHD in cardiovascular department of Yantai Yuhuangding Hospital from December 2015 to January 2018.Forty patients with coronary heart disease without heart failure were randomly selected as group B.Forty patients with coronary heart disease with NYHA? were randomly selected as group C1,and 40 patients with coronary heart disease with NYHA ? were randomly selected as group C2,randomly selected.40 patients with coronary heart disease with NYHA IV heart were in group C3,and 40 healthy people randomly selected in Yantai Yuhuangding Hospital were group A.Twenty-two patients with effective treatment in group C were selected as group C4.All patients in group C4 were examined for BNP and CA125 more than once.CA125 and BNP were measured within 12 hours.All the patients undertook echocardiography.SPSS20.0 statistical software was used to analyze the relevant data.The skewed distribution data is represented by the median.The Shapiro-Wilk method was used to test the normality,the one-way ANOVA was used to compare the normal distribution measurement data,the non-parametric rank sum test was used to compare the skewed distribution data,and the chi-square test was used to compare the count data.The correlation coefficient between serum CA125 concentration and serum BNP concentration and echocardiographic parameters was multivariate correlation analysis(PERASON method).The relationship between ten laboratory measurements of TC,HDL-C,LDL-C,FBG,Cr,CA125,BNP,LVEF,LVEDD,FS was multivariate logistic regression.The diagnostic value of CA125 as an independent index and CA125 combined with BNP in diagnosis of heart failure was analyzed by ROC curve method.P<0.05 was considered statistically significant.Results:The levels of CA125 in group C were higher than those in groups A and B.The higher the level of heart function,the higher the levels of serum CA125,BNP,and LVEDD,and the lower the levels of LVEF.There was a positive correlation between CA125 and BNP(r=0.412,p<0.001),and a negative correlation between LVEF and FS(r=-0.185,r=-0.196,p<0.05).Moreover,CA125 and BNP were independent risk factors(OR>1),but there was no significant correlation between other indicators and disease(P>0.05).The best diagnostic cut-off point for CA125 in coronary heart disease was 19.455 U/mL.In the combined diagnosis of heart failure with CA125 and BNP indexes,the diagnostic cut-off point for CA125 was 11.38 U/mL and BNP was 242 pg/ml.With the rank sum test(before and after CA125 treatment,Z=-4.041,p=0.000<0.05;before and after BNP treatment,Z=-5.096,p=0.000<0.05),the difference of CA125 and BNP after treatment was significant.Conclusion:The levels of CA125 are closely related to coronary heart failure.There are significant differences in CA125,BNP,LVEF,LVEDD,and FS among the three group.The higher the levels of heart function,the higher the levels of serum CA125,BNP,and LVEDD,and the lower the levels of LVEF and FS.There was a positive correlation between CA125 and BNP and a negative correlation with LVEF and FS respectively.CA125 can be used as a biomarker forjudging heart failure in patients with coronary heart disease.According to the ROC curve,it can be concluded that CA125 is of great significance for the detection of coronary heart failure.The best diagnostic cut-off point is 19.455U/mL.CA 125 and BNP are two indicators combined to diagnose heart failure,and the two indicators CA125 and BNP are jointly diagnosed.In heart failure,the diagnostic cut-off point for CA 125 was 11.38 U/mL and BNP was 242 pg/ml.On the one hand,C A125 combined with BNP can improve the sensitivity and specificity of diagnosis of heart failure in patients with coronary heart disease.On the other hand,with the improvement of cardiac function,CA125 and BNP both showed a downward trend.CA125 and BNP have similar accuracy,and CA125 is expected to have to become a more convenient and inexpensive evaluation of cardiac function in patients with coronary heart disease complicated by chronic heart failure.
