BackgroundFujian Province is located on the southeast coast.The rapid development of society and economy in the past 20 years has brought about a massive change of the disease patterns.Although Fujian Province has got the results of death cause monitoring,it is only the relevant data at the provincial level,and there are no relevant indicators of the municipalities so far.Besides,with only mortality rate,it's not enough to fully reflect the impact of disease on residents' health..Disease burden is a comprehensive indicator and a tool for measuring the degree of health hazard of a disease.It can standardize the impact of different diseases on the health of the population,and achieve direct comparison.Related studies have shown that disease burdern varies widely in each province.The 9 municipalities in Fujian Province are located in different geographical environments and have different levels of economic development.Whether there is a heterogeneity in the burden of disease such as death patterns and life expectancy among municipalities,there is still a lack of research.ObjectivesTo grasp the data of death patterns and life expectancy of residents in the municipalities of Fujian Province in 2015 and to identify the main diseases affecting the health level of residents in different municipalities so as to provide scientific basis for formulating health policies and allocating health resources rationally according to local conditions.MethodsThis study adopts the GBD 2015 research method,systematically analyzes the data of death cause monitoring in Fujian Province,and conducts analysis of the municipality level to produce indicators such as all-cause mortality rate,cause-specific mortality rate,years of lif'e lost due to premature death(YLL).years lost due to disability(YLD).disability-adjusted life year(DALY),early death probability rate and life expectancy.This study is mainly for quantitative analy'sis,using SAS 9.4.Stata 14.0.R 3.4.4.AreGIS 10.2 software for data collation and analysis.OutcomesIn 2015.the death rate of residents in Fujian Province was 628.02/100,000,and the standard mortality rate was 635.39/100,000.Among the 9 municipalities,the lowest standard mortality rate was in Xiamen(488.21/100,000),and the highest was in Ningde(695.64/100,000).Chronic non-communicable diseases accounted for more than 80%of total deaths.Male mortality rate was significantly higher than female mortality rate.The top 10 causes of death were cerebrovascular disease,ischemic heart disease,chronic obstructive pulmonary disease,lung cancer,liver cancer,gastric cancer,hypertensive heart disease,esophageal cancer,road injury,Alzheimer's disease and other dementias.The top 10 causes of death in the 9 municipalities were basically the same as those in the whole province.with only slight difference in rank..Among the top 20 causes of death,cancers included lung cancer,liver cancer,gastric cancer,esophageal cancer,colorectal cancer,breast cancer,pancreatic cancer,nasopharyngeal cancer,and the total standard mortality rate was 150.70/100,000.In 2015,the resident standard YLL,YLD.and DALY rates of Fujian Province were 12,598.28/100,000,9,715.22/100.000 and 22.313.51/100.000,respectively.The lowest standard YLL,YLD.and DALY rates among the 9 municipalities were in Xiamen,followed by Fuzhou.The highest rate of standard YLL was in Ningde and the highest rate of standard YLD and DALY was in Zhangzhou.Male standard DALY rate was significantly higher than that of female,.The loss of DALY caused by chronic non-communicable diseases accounted for the main proportion.The top 10 causes of death affecting DALY were cerebrovascular disease,lower back and neck pain,ischemic heart disease,sensory organ disease,chronic obstructive pulmonary disease,road injury,liver cancer,lung,tracheal and bronchial cancer,gastric cancer and congenital malformations.Cerebrovascular disease ranked the first in standard DALY rate in all the municipaties except for Xiamen(lower back and neck pain ranked the first and cerebrovascular disease the third).In 2015,the premature death probability rate caused by the four types of chronic diseases in Fujian was 16.09%,with male 21.89%and female 9.45%.Male was 2.32 times as many as female..Among then,the premature death due to tumors acctounted for 62.83%,while cardiovascular disease for 32.50%..Among the 9 municipalities,Xiamen had the lowest premature death probability rate(10.97%),Fuzhou the second lowest(15.14%),and Longyan the highest(18.04%).In 2015,the life expectancy of residents in Fujian Province was 77.04 years old,of which male 73.82 years old and female 81.17 years old.,with female signifcantly longer than male.Among the 9 municipalities,Xiamen has the longest life expectancy(80.20 years old),Fuzhou the second(77.86 years old),and Ningde the lowest(75.92 years old).The gap between healthy life expectancy and life expectancy of the residents in Fujian was 8.65 years.The smallest gap was in Xiamen,and the largest was in Zhangzhou.ConclusionThe disease burden of residents in Fujian Province is slightly lower than the national average The main diseases affecting the province and municipalities are chronic non-communicable diseases.There are differences in death patterns and life expectancy among different municipalities.The disease burden of economically underdeveloped municipalities is higher than that of economically developed.Local governments should intervene in key chronic diseases such as cerebrovascular disease,ischemic heart disease,cancer,COPD,diabetes mellitus,hypertension,and road traffic injuries according to local conditions.Meanwhile,the intervention of behavioral risk factors such as diet and tobacco should be strengthened.
