Background: Facing up with the increasing menace of chronic diseases and emerging and reemerging infectious diseases, how to allocate the limited health resources is always a common serious problem to WHO and each nation. The Study of burden of disease (BOD) provides the important evidence for setting priorities and emphases in health research , and is in favor of optimizing the allocation of health resources and improving human's quality of life. Valid health statistics are required for policy-making and taking essential measures. With the development of society, the sequence of disease and cause-of-death are changing. People have realized that health outcomes are not only fatal but non-fatal situation (including health and illness) and patients are disabled in different degrees. Traditional indexes such as mortality rate, prevalence or incidence are limited and unilateral and can not value how severe diseases' effect on health. So more sensitive measures are needed to study overall health even BOD.Objective: Using disability-adjusted life years (DALY) and Disability-adjusted Life Expectancy (DALE) , two indicators presented recently, the study aims to develop a method for evaluating BOD for the residents in Shilin county, and evaluate their health status from different aspects with several indicators, and provide scientific guide for rural dweller's diseases prevention and treatment, and provide reference for reasonable application of these indicators through comparing the new and old indicators.Methods: The all-causes-of- death survey data in Shilin county in 2004 were used . DALY approach which recommended by WHO and my modified equation were used .With PPS(sampling with probability proportional to size) sampling method, rural dwellers from some villages were surveyed by field investigation and DALE approach of Sullivan was used. Then , these two indicators were used to evaluate and analyze BOD of various diseases.Results: (1)The five leading causes of YLL(Years of lost) in descending order: Respiratory diseases. Unintentional injuries, Cardiovascular diseases, Intentional injuries, Malignant neoplasms .(2)Diseases burden probability of I kind or II kind was slightly lower than CHN ,but the result of III kind was opposite .
