Purpose:To study the correlation between fire-depression theory,fire-depression theory and TCM differentiation and treatment of primary trigeminal neuralgia, the basis of fire-depression diagnosis and the law of clinical differentiation and treatment, and to effectively guide the clinical differentiation and treatment of primary trigeminal neuralgia by using fire-depression theory. Methods: Literature research and clinical research are combined, and clinical case investigation is combined.(1)The theory of fire-depression is studied. The concept, origin, etiology, pathogenesis, clinical manifestation and treatment of fire-depression are studied from the aspects of theoretical origin, development, supplement and exploration in modern and contemporary times by sorting out the related discussions of fire-depression theory in ancient and modern literature.(2)Periodical literature research, collected nearly 10 years of Chinese medicine treatment of trigeminal neuralgia related periodical literature, extract cases reported in the periodical, analysis of the proportion of cases of trigeminal neuralgia treated by fire depression theory. In order to guide the treatment of trigeminal neuralgia, the correlation between fire depression theory and trigeminal neuralgia was discussed.(3)In clinical research,60 patients with primary trigeminalneuralgia were selected to observe their clinical symptoms, symptoms and changes of their condition. The relevant factors of the selected cases were analyzed statistically. The correlation between symptoms, signs and fire depression was studied. The classification, treatment methods and prescriptions of fire depression theory in the differentiation and treatment of primary trigeminal neuralgia were studied.Result:(1)The diagnosis of fire depression was based on five main symptoms and nine secondary symptoms.(2)There are five main symptoms in the diagnosis of fire depression according to the diagnostic criteria, which are as follows:(1)Face red and stagnant;(2)Red or red tongue,Less moss or yellowish moss or thin moss with less zinc,Even the tongue is dry and cracked;(3)Pulse sink number or sink late or chord number or impetuous number;(4)Swollen,painful,or knife-cut,or burning sensation;(5)Bitter or sticky, thirsty to drink cold; There were 9 secondary symptoms, which are as follows:(1)Be agitated and irritable;(2)Dizziness or blindness, tinnitus, insomnia and dreaminess;(3)Dry throat, sore throat, swollen gums or sore mouth and tongue;(4)Stool bowel, yellow urine, irritable urine, history of haemorrhoids or spicy food is bloody stool;(5)When it is hot,it is dramatic, but when it is cold,it is slow.It is induced by cold, wind, anger and other incentives;(6)Hypochondriac rib pain or burning pain, abdominal distension and abdominal pain, poor acceptance;(7)The limbs are not warm,or even cold;(8)No sweat or head sweating and body sweating;(9)Mental anxiety or delirium,or even fainting;(3)The proportion of primary trigeminal neuralgia that accords with the pathogenesis of fire depression accounted for 55.17% in the periodical literature study, and 65.00% in the clinical case investigation study. Therefore, fire depression is the key pathogenesis of some primarytrigeminal neuralgia.(4)The theory of fire depression guides the classification of the treatment of primary trigeminal neuralgia, which can be divided into four types: Xiaochaihu Decoction of Cold Wind and Depression of Fire,Huanglian Wendan Decoction of Phlegm Depression and Heat Dispersion,Sanjiao Dispersion of Depression of Fire,Sanjia Dispersion of Yin Deficiency,Fire Depression and Wind Movement. Conclusion: The theory of fire depression has a significant correlation with primary trigeminal neuralgia, and the application of fire depression theory can effectively guide the clinical differentiation and treatment of primary trigeminal neuralgia.
