Objective: To observe the clinical efficacy and safety of YaJia decoction in the treatment of subacute thyroiditis of heat toxin Yongsheng type.Methods: We conducted a randomized,controlled,open-label trial involving hospitalized adult patients with confiremed subacute thyroiditis with heat toxic dampness type.Patients underwent randomization were assigned in a ratio to receive either YaJia decoction(200ml,thrice a day for 8 weeks),in addition to standard care,or standard care alone.Triiodothyronine,tetraiodothyronine,thyrotropin,serum creatinine,urea nitrogen,erythrocyte sedimentation rate,fever abating time,pain relief time,and clinical cure rate were observed in the two groups.The primary end point was the time to clincal improvement,defined as the time from randomization to either an improvement of fever abating time or pain relief time.Patients in the two groups were followed up 3 months,and the recurrence of the patients in the two groups was observed.Result:1.The control group was effective in 10 cases(43.48%),markedly effective in 5 cases(21.73%)and clinically cured in 8 cases(34.78%).The clinical cure rate in the trial group was significantly higher than that in the control group(p<0.05).2.The pain relief time shorter than that of the control group(mean difference,1.41;95% Confidence interval(-2.69 to-0.13);The fever abating time of the test group shorter than the control group(mean difference,-1.27;95% Confidence interval(-2.31 to-0.24).3.There was no significant difference in FT3,FT4,ESR,TSH,Scr,BUN,AST and ALT between the two groups before and after treatment.4.Before treatment,the median of TCM symptoms in the test group was 21.00 and that in the control group was 20.00,there is no significant statistical difference in two groups.After treatment,the median of TCM symptoms in the test group was 1.00,and the median of total score of the control group was 4.00,and the difference was statistically significant.5.After three months,there were 5 cases(21.73%)of hypothyroidism in the control group and 2 cases(9.52%)in the experimental group,but there was no statistical significance between the two groups.6 cases(26.08%)recurred in the control group and 2 cases(9.52%)in the experimental group.The comparison between the two groups was no statistically significant too.Conclusion: YaJia decoction is superior to the control group in relieving fever and pain and relieve the TCM clinical symptoms of heat-toxic Yongsheng subacute thyroiditis obviously.
