Survival Analysis On Stage Ⅲ-Ⅳ Gastric Cancer Under Combined Treatment Of Traditional Chinese And Western Medicine

Object iveTo discuss the relationships between the prognosis of chemotherapy regimens containing 5-Fluorouracil or Capecitabine for patients with stageⅢ-Ⅳ gastric cancer and gender,age,TNM stage of tumor,lesion location,degree of pathological differentiation and time of combined treatment of traditional Chinese medicine,etc.MethodsConduct the following-up visit to the gastric cancer cases with combined traditional Chinese medicine and chemotherapy containing 5-Fluorouracil or Gemcitabine at the Cancer Center of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2012 to June 2017;select the indexes of gender,age,TNM stage of tumor,lesion location,degree of pathological differentiation and time of combined treatment of traditional Chinese medicine,etc.;know the patients’ progression-free survival and overall survival;analyze the main factors affecting the patients’ prognosis.ResultsAs for the group that takes traditional Chinese medicine for a long term,the median overall survival of is 39.378 months,and 95%of confidence intervals are 30.244-48.513;as for the group that takes traditional Chinese medicine for a short term,median overall survival of is 24.125 months,and 95%of confidence intervals are 20.343-27.907;the inspection result of Log Rank is P<0.05,thus the difference was statistically significant.Six-month survival rates of these two groups are 100%and 97.7%;one-year survival rates are 88.9%and 86.9%;two-year survival rates are 75.3%and 75.3%;three-year survival rates are 34.2%and 27.4%.Therein,the six-month,one-year and three-year survival rates of the group that takes traditional Chinese medicine for a long term are higher than those of the group that takes traditional Chinese medicine for a short term;and the two-year survival rates of the two groups are the same.As for the group that takes traditional Chinese medicine for a long term,the median progression-free survival is 12.774 months,95%of confidence intervals are 9.520-16.027;as for the group that takes traditional Chinese medicine for a short term,the median progression-free survival is 7.483 months,95%of confidence intervals are 5.918-9.048;the inspection result of Log Rank is P<0.05,thus the difference was statistically significant.Six-month progression-free rates of these two groups are 68.3%and 47.6%;one-year progression-free rates are 37.5%and 16.3%;two-year progression-free rates are 13.4%and 0%.Therein,the six-month,one-year and two-year progression-free rates of the group that takes traditional Chinese medicine for a long term are higher than those of the group that takes traditional Chinese medicine for a short term.The differentiation degree is one of the risk factors affecting the prognosis of patients with gastric cancer and the relative risk of such factor is 1.228,prompting that the mortality risk of low differentiation is 1.228 times of other differentiated cancers.Besides,T stage is also one of the risk factors and the relative risk of such factor is 1.168,prompting that the tumor infiltration depth is deep and its mortality risk is high.Taking traditional Chinese medicine for a long term is one of the factors affecting the prognosis,its P<0.05,having statistical significance;the regression coefficient is-1.000,prompting that taking traditional Chinese medicine for a long term is a favorable factor of prognosis;the relative risk is 0.368,prompting that taking traditional Chinese medicine for a long term can relatively reduce 36.8%mortality risk,comparing with the group that takes traditional Chinese medicine for a short term.Conclusion(1)Combined treatment of traditional Chinese and Western medicine benefits the patients with stage Ⅲ-Ⅳ gastric cancer.The long-term intervention of traditional Chinese medicine can prolong the survival time and the median progression-free survival,increasing the survival rate and progression-free survival rate;(2)The analysis result of multifactorial prognosis shows that:T stage,degree of pathological differentiation and intervention time of traditional Chinese medicine are risk factors and protective factor affecting the patients’prognosis;wherein,the intervention time of traditional Chinese medicine isthe protective factor,and increasing the intervention time of traditional Chinese has medicine positive effects to the prognosis;higher T stage and poorly differentiated carcinoma are risk factors of the prognosis of patients with gastric cancer.
