| Objective : To explore the role of tonifying primordial Qi,detoxifying,unblocking circulation tracts and guiding evil methods in the treatment of Diabetic renal failure(deficiency of spleen and kidney Yang with stasis syndrome).Using Bu Yuan Xiao Ke Shen Shuai an decoction and external enema with traditional Chinese medicine.Comparative observation,comprehensive evaluation of its efficacy and safety.Methods : A total of 72 patients diagnosed as Diabetic renal failure(deficiency of spleen and kidney Yang with stasis syndrome)(stage V DN)were collected from outpatient clinics or included in the endocrinology department.They were arbitrarily divided into a therapy group and a contrast group of 36.The therapy group was given Bu yuan Xiaoke Shen shuai an Decoction orally,with Chinese medicine enema formula for external use.The contrast group was given Niaoduqing granules(sugar-free type)and Haikun Shenxi capsules orally.At the same time,both groups of patients were given the symptomatic basic treatments such as lowering blood sugar,lowering blood pressure,lowering blood lipids,and correcting acidosis.Since the date of observation,patients have been required to adhere to a high-quality low-protein,low-salt,low-fat diet.In the case observation,pay attention to the changes in the symptoms of the patients before and after treatment,the relevant physical and chemical examination results,and whether there are adverse reactions.And finally,analyze and evaluate them by statistical software.Results:1.During the period from 4 weeks to 12 weeks of observation of the medical records,3 cases were lost in the therapy group and 5 cases were lost in the contrast group.No adverse reactions were found in the two groups,or the patients withdrew from the experiment due to adverse reactions.There were 33 cases in the case therapy group with a overall response rate of 81.82% and 31 cases in the contrast group with a overall response rate of 61.29%.The difference was statistically significant(P<0.05).2.Compare the scores of TCM symptoms among two groups of patients after treatment.The clinical symptoms of the therapy group were obviously better than before(P<0.05),and the improvement in chilling limbs and fatigue was significantly superior to the contrast group(P<0.01),the improvement of symptoms such as facial and foot edema,thin stools,poor appetite,frequent nocturia,vomiting,and abdominal distension were obviously different from those in the contrast group(P<0.05).However,there was no obvious improvement in waist and knee soreness compared with the contrast group(P>0.05).3.In terms of reducing Scr and BUN,the Scr level in the therapy group decreased obviously after treatment(P<0.01),and was better than the contrast group(P<0.05).At the same time,the decrease of BUN in the therapy group was significantly different than that in the contrast group(P<0.01).4.In the qualitative evaluation of urinary protein,the efficacy of the therapy group was obviously better than that of the contrast group(P<0.01),and the quantitative decrease of urinary protein at 24 hours was also statistically significant(P<0.05).5.Changes in the glomerular filtration rate,the GFR of the therapy group was obviously improved after treatment,and the improvement rate was superior to the contrast group(P<0.05).Conclusion:Tonifying primordial Qi,detoxifying,unblocking circulation tracts and guiding evil methods for the treatment of Diabetic renal failure(deficiency of spleen and kidney Yang with stasis syndrome),compared with the conventional treatment of Niaoduqing granules and Haikun Shenxi capsule,can significantly improve the clinical symptoms and reduce serum creatinine,urea nitrogen,24 h urinary protein quantification,slow down the leakage of urinary protein,and improve the glomerular filtration rate significantly better than the contrast group,with reliable curative effect and clinical application safety. |
