Objective Recent studies have shown that benign prostatic hyperplasia(BPH)is associated with abnormal high body mass index(BMI)and metabolic diseases such as hypertension,but the relationship between(BPH)and BMI is still unclear.Obesity(or overweight),as the primary diagnostic factor of metabolic syndrome(MS),is the main risk factor of hypertension,diabetes and other diseases in its components.Based on metabolic classification and excluding age and other confounding factors,the prostate volume(PV),prostate specific antigen(PSA)level,prostate density(PSAD)and prostate mass(PSAM)in patients with abnormal high BMI with different metabolic diseases were analyzed to investigate the relationship between abnormal high BMI and BPH.Methods The clinical data of 1261 patients with benign prostatic hyperplasia admitted to the second affiliated Hospital of Suzhou University from January 2016 to September 2018 were retrospectively collected.All patients were Han adults,excluding non-first admission patients and patients complicated with prostatitis,urinary tract infection,urinary retention and other factors affecting the level of PSA.After screening,a total of 973 cases were included in the study.Through the collection of patients’ height and weight,basic diseases,biochemical indicators,image information and other related clinical data.PV,PSA,PSAD and PSAM,of BMI abnomaly patients with different metabolic diseases were compared to evaluate the relationship between abnormal high BMI and BPH in patients with different metabolic diseases.The value of BMI in the occurrence and progress of BPH was obtained.Results(1)General situation:973 cases were included in the study,including 48 patients in the thinning group,462 in the normal group,368 in the overweight group,and 95 in the obese group.The mean age was 71.09±0.51,the mean PSA was 7.65±0.47 ng/ml,the PV was 76.61±2.42 ml,the mean of PSAD was 0.1010±0.0052ng/(ml*cm3),the mean of PV was 76.61±2.42 ml,the mean of PSAD was 0.1010±0.0052ng/(ml*cm3).The mean value of PS AM was 23.30±1.44μg.(2)According to the BMI interval,the P values of PV and PSAD were less than 0.05 in normal and above BMI patients,and there was significant difference between the two groups(P<0.05).The P value of PSA and PSAM was more than 0.05,there was no significant difference.The results showed that the P value of PV was not complicated with metabolic disease,but the P value of PSA and PSAD was only associated with hypertension.The P value of PV in normal BMI patients and overweight and obese patients with more than two metabolic diseases was less than 0.05,and the difference was statistically significant.P value of PSA,PSAD and PSAM without metabolic disease,P value of PV and PSAM only in hypertension,P value of PSA,PV,PSAD and PSAM in normal and overweight patients with diabetes mellitus,PSA,in normal and overweight patients with hyperlipidemia only.The P value of PV,PSAD and PSAM,the P value of PSA,PSAD and PSAM in patients with more than two metabolic diseases,and the P value of PV in overweight and obese patients with more than two metabolic diseases were all higher than 0.05.There was no significant difference between the two groups.(3)There was no significant difference in the P value of PSA,PV,PSAD and PSAM between the patients with any metabolic disease and the body mass index patients(P>0.05),and there was no significant difference between these groups.Conclusion The age of BPH was significantly higher in the patients with emaciation.BMI was the independent factor of BPH and had a significant effect on the growth of PV.When combined with metabolic diseases,it does not affect the overall effect of BMI on BPH.When high BMI was combined with hypertension,its negative effect on PSA was enhanced,but hyperglycemia and hyperlipidemia had no effect.When high BMI is associated with arbitrary metabolic diseases,the difference in PV among the groups treated at first admission will be weakened.
