?Objective?This study aims to analyze and discuss the risk factors of colorectal polyp formation and recurrence by comparing the general characteristics of patients in the control group without polyp and the colorectal polyp group,the recurrence group and the non-recurrence group,so as to find out the high-risk population of intestinal polyp formation and recurrence through relatively common clinical factors and conduct good supervision.?Methods?In this study,a total of 242 patients with colorectal polyps and 24 control patients without polyps were included,46 patients with recurrent polyps and 40 patients without recurrent polyps.General data of patients between the groups were compared,including: gender,age,body mass index(BMI),blood lipid,number of intestinal polyps,size of intestinal polyps,location of intestinal polyps,histological type,complications,smoking and drinking history,and multivariate logistic regression analysis was performed.ROC curve was used to predict the critical value,specificity and sensitivity of the related factors of intestinal polyp formation and recurrence.? Results ? The age,BMI,low-density lipoprotein cholesterol(LDL-C)and apolipoprotein B(Apo B)of the polyp group were higher than those of the control group(51.55±11.92 vs 43.75±13.64,23.70±3.40 vs 20.74±3.30,3.32±0.91 vs 2.52±0.58,0.95±0.26 vs 0.81±0.15,P<0.01),triglyceride(TG)levels were higher in patients with polyps than in controls(1.58mmol/L vs 1.09mmol/L).And the high-density lipoprotein cholesterol(HDL-C)of the polyp group was lower than that of the control group(1.30±0.31 vs 1.48±0.36,P<0.01).The age and triglyceride of the relapsed group were higher than that of the non-relapsed group(57.37±9.42 vs 47.95 ±11.925,1.90±1.57 vs 1.27±0.70,P<0.05),and the HDL-C of the relapsed group was lower than that of the nonrelapsed group(1.20±0.30 vs 1.36±0.35,P<0.05).The sensitivity and specificity were 75.2% and 58.3%(AUC=0.681)when the age threshold between the polyp group and the control group was 43.5 years old.When BMI threshold is 20.54 kg/m^2 to distinguish polyps group and control group,the sensitivity was 81.9%,specificity was 54.2%(AUC = 0.728).When the critical value of HDL-C between the polyp group and the control group was 1.325 mmol/L,the sensitivity and specificity were 56.6% and 83.3%,respectively(AUC=0.664).When the critical LDL-C value of the polyp group and the healthy group was distinguished by 2.965 mmol/L,the sensitivity was 64.0% and the specificity was 83.3%(AUC=0.772).When the critical Apo B value of the polyp group and the healthy group was distinguished by 0.975g/L,the sensitivity and specificity were 43.0% and 91.7% respectively(AUC=0.681).The sensitivity and specificity were 87.0% and 50.0%(AUC=0.737)at the age threshold of 47.5 years for the relapsed and nonrelapsed groups.When 1.14 mmol/L was used to distinguish the TG critical value between the relapsed group and the non-relapsed group,the sensitivity was 73.9% and the specificity was 60.0%(AUC=0.690).The sensitivity and specificity were 56.5% and 67.5%(AUC=0.624)when 1.21 mmol/L was used to distinguish the HDL-C critical value between the recurrent group and the non-recurrent group.And logistic regression indicated that age(OR=1.763),decreased HDL-C level(OR=8.203),and increased LDLC level(OR=9.579)were independent risk factors for polyp formation(P<0.05).For males(OR=5.601),polyps of multiple sites(OR=8.026),interval > 3 years(OR=7.777),and TG(OR=3.883)were independent risk factors for polyp recurrence(P<0.05).?Conclusions?Patients with age > 43.5 years,HDL-C <1.325 mmol/L and LDLC >2.965 mmol/L are at risk of polyps grow,which requires strengthened supervision.However,men,multi-site polyps,long interval review and elevated TG level are independent risk factors for the recurrence of intestinal polyps.For male patients with multi-site polyps,combined with TG> 1.14 mmol /L,the interval for colonoscopy review can be shortened appropriately.
