ObjectiveTo investigate and understand the level of hope,negative emotions and fatigue in patients with permanent enterostomy;To explore the correlation between hope level and fatigue,negative emotions and fatigue of patients with permanent enterostomy,and analyze the related factors of fatigue;To explore the effect of nursing intervention based on Snyder hope theory on negative emotion and fatigue in patients undergoing permanent enterostomy.MethodsThis research is divided into two phases.The first stage: A questionnaire survey was performed on 76 permanent enterostomy patients who were treated in the general surgery and stoma clinic of a tertiary class hospital in Zhenjiang from November 2018 to March 2019.The survey tools were the General Information Questionnaire,the Chinese version of Multidimensional Fatigue Index(MFI-20),the Herth Hope Scale,and the Positive and Negative Affect Schedule(PANAS).SPSS20.0 software was used to perform descriptive analysis,analysis of variance,and multiple linear regression analysis on the obtained data to reflect the current status of permanent enterostomy patients' fatigue and other related factors.The second stage: From March 2019 to October 2019,68 patients with permanent enterostomy who met the inclusion criteria in a general surgery in a tertiary hospital of Zhenjiang City were selected as the research objects,and 34 cases in the first and second general surgery departments were used as the experimental group and the control group.The control group performed routine nursing interventions for patients with permanent enterostomy,the experimental group adopted nursing intervention based on Snyder's hope theory beside conventional nursing,and patients' levels of hope,negative emotions,and fatigue were assessed before and 3 months after the intervention.Results1.The permanent enterostomy patient's total hope level was(34.38±1.77)points;the negative emotion score was(24.13±8.17)points;The total score of fatigue was(58.51±11.86)points,and the items of psychological fatigue,physical fatigue,and mental fatigue were(3.63±0.67)points,(3.38±0.73)points,and(2.45±0.90)points respectively.2.The results of single factor analysis showed that there were statistically significant differences in fatigue scores of patients with permanent enterostomy in postoperative time,degree of self-care,sleep,and exercise time(P <0.05).3.The results of correlation analysis showed that hope level was negative with total fatigue score and dimensions(psychological fatigue,physical fatigue,mental fatigue)(r =-0.476,r =-0.502,r =-0.468,r =-0.544,P <0.05);negative Emotion was positively correlated with total fatigue score and various dimensions(r = 0.931,r =0.727,r = 0.861,r = 0.875,P <0.01).4.The results of multiple stepwise regression analysis showed that self-care,sleep,exercise time,hope level,and negative emotions are important factors that affect the fatigue of patients with permanent enterostomy.5.After the intervention,the hope score of the two groups of permanent enterostomy patients increased,the hope score of the experimental group was higher than the control group's,and the difference between two groups was statistically significant(P <0.05).After the three months intervention,the negative emotion and fatigue scores of the experimental group were lower than those of the control group,and the differences between two groups were statistically significant(P <0.05).The difference was statistically significant in physical fatigue,so that in psychological fatigue(P <0.05),and there was no significant difference in mental fatigue(P> 0.05).Conclusions1.The level of hope,negative emotion,and fatigue of patients with enterostomy are generally at intermediate levels;2.The level of fatigue of patients with permanent enterostomy is affected by many factors,among which self-care,sleep,exercise time,hope level,negative emotion are the main factors affecting fatigue;3.The implementation of nursing measures based on Snyder's hope theory cansignificantly improve the hope level of patients with permanent enterostomy,relieve negative emotions and fatigue,and thus promote their physical and mental health.
