ObjectiveThe clinical efficacy of the treatment of occipital neuralgia was evaluated by observing bloodletting at the “occipital three-point” combined with electroacupuncture of the Cervical Jiaji acupoints,compared with conventional acupuncture.It’s supposed to provide a basis for clinical application.MethodsThe 60 patients with the occipital neuralgia were randomly divided into a treatment group bloodletting at the “occipital three-point” combined with electroacupuncture of the Cervical Jiaji acupoints)and a control group(conventional acupuncture group),with 30 cases in each group.Acupoint selection in treatment group: occipital point-1,occipital point-2,occipital point-3,C2 Jiaji point,C3 Jiaji point.According to the patient’s headache,select the acupuncture points,hold a disposable blood collection needle to quickly puncture,the acupuncture depth is about 2-3mm,repeatedly squeeze and relax the pinhole alternately,and squeeze out about 1ml of blood.When acupuncturing Jiaji points,twisting and laxative method is used.After getting gas,connect the electroacupuncture electrodes to C2 and C3 Jiaji points respectively,select sparse and dense waves,frequency conversion square wave with a frequency of 2Hz/100 Hz,the amount of stimulation can be Tolerance is degree,treatment for 30 min.In the control group,bilateral Tianzhu,Houding,Fengchi,Wangu,Houxi were selected,and the method of restoring and relieving the diarrhoea was performed,and the needle was kept for 30 minutes after getting gas.The treatment in both groups was once a day,7 times as a course of treatment,a total of 1 week of treatment.In the study,the Visual Analogue Scale(VAS)score and its reduction rate were the main outcome indicators.The modified headache score scale,Headache Impact Test-6(HIT-6),Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS)and the incidence of adverse reactions were secondary outcome indicators,and their efficacy and safety were evaluated.ResultsA total of 60 patients were included in this study.One patient in the treatment group fell off due to work and 59 patients completed the treatment.Before treatment,the gender,age,course of disease,pain location,VAS,headache score,HIT-6,SAS,and SDS of the two groups of patients were not statistically significant(P>0.05),and were comparable.1.Visual Analogue Scale(VAS): After one treatment,the VAS score of the treatment group was lower than that before treatment,which was statistically significant(P<0.05),and there was no significant difference in the VAS of the control group than before(P>0.05).After three treatments,the VAS scores of the two groups were lower than before(P<0.05),and the treatment group was better than the control group(P<0.05).After the treatment,the VAS scores of the two groups were improved compared with those before treatment,with statistical difference(P<0.05);compared between the groups,the reduction of VAS scores of the treatment group was better than that of the control group(P<0.05).It is suggested that bleeding at the “occipital three-point” combined with electroacupuncture treatment can effectively reduce the patient’s pain level,and it has a faster onset,which is significantly better than conventional acupuncture treatment.2.Modified headache score scale: after one treatment,the number of pain episodes and the degree of pain in the treatment group were reduced compared with those before treatment,which was statistically significant(P<0.05);the total score of headache scores in both groups was significantly more than before treatment Decreased(P<0.05),and the headache score of the treatment group was significantly lower than that of the control group(P<0.05).After 3treatments and after treatment,the comparison between the treatment group and the treatment group was superior to the control group in improving the number of pain episodes,duration,and degree of pain(P<0.05);the total score of headache scores in both groups was significantly lower than before treatment(P<0.05),and the headache score of the treatment group was significantly lower than that of the control group(P<0.05).It is suggested that bleeding at the “occipital three-point” combined with electroacupuncture treatment of occipital neuralgia can effectively reduce the number of pain attacks,shorten the duration of pain,and reduce the degree of pain,and it is better than the conventional acupuncture group.3.Headache Impact Measurement Scale(HIT-6): After one treatment,the comparison between the two groups was significantly different than before(P<0.05),and the HIT-6 score was not statistically significant between the two groups(P>0.05).After three treatments,the HIT-6 scores of the two groups were lower than before,with statistical significance(P<0.05),and the treatment group was significantly better than the control group(P<0.05).After treatment,the HIT-6 scores of the two groups were significantly lower than before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).It is suggested that bloodletting of occipital three-point combined with electroacupuncture treatment can significantly improve the quality of life of patients with occipital neuralgia.4.Self-rating Anxiety Scale(SAS): After one treatment,the SAS score of the treatment group was significantly lower than before treatment(P<0.05),and the SAS score of the control group was not significantly different than before treatment(P>0.05).After treatment,the SAS scores of the two groups were significantly improved compared with before treatment(P<0.05).Compared between groups,the SAS score of the treatment group was better than that of the control group(P<0.05).It is suggested that bloodletting of occipital three-point combined with electroacupuncture is better than conventional acupuncture in alleviating the anxiety of patients,and has rapid effect.5.Self-rating Depression Scale(SDS): After one treatment,the SDS score of the treatment group was significantly lower than before treatment(P<0.05),and the SDS score of the control group was not significantly different than before treatment(P>0.05).After treatment,the SDS scores of both groups were significantly lower than before treatment(P<0.05),and the treatment group was significantly better than the control group(P<0.05).It is suggested that bloodletting of occipital three-point combined with electroacupuncture treatment is better than conventional acupuncture in improving patients’ depression mood,and has rapid effect.6.During the study,the incidence of adverse reactions in the treatment group was 6.9%and that in the control group was 3.3%.There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).7.After the treatment,the cure rate of the treatment group was 82.8% and the total effective rate was 96.6%;the cure rate of the control group was 56.7% and the total effective rate was 83.3%.There was no statistically significant difference in the total effective rate between the two groups(P>0.05).There was a significant difference in the recovery rate between the two groups(P<0.05),suggesting that the recovery rate of the occipital three-point bloodletting combined with electroacupuncture group was better than the conventionalacupuncture group.Conclusion1.The treatment of occipital three-point bloodletting combined with electroacupuncture of the Cervical Jiaji acupoints for occipital neuralgia is significantly better than conventional acupuncture treatment,and has rapid effect.2.The treatment of occipital neuralgia with bloodletting at the occipital points combined with electroacupuncture of the Cervical Jiaji acupoints can significantly reduce the times of headache attack,duration and pain,effectively improve the patient’s quality of life,and relieve patients’ anxiety and depression.It is a safe and effective treatment.
