Objective To explore the relationship between anti apolipoprotein A1 antibody(anti apoA-lIgG),metalloproteinase9(MMP-9),paraoxonasel(PON-1),oxygenized low density lipoprotein(oxLDL),high density lipoprotein(HDL)and carotid atherosclerosis in patients with acute cerebral infarction.Method We select 120 cases of acute cerebral infarction in my department,and use color doppler ultrasound to check the patients' cervical blood vessels.According to the degree of carotid atherosclerosis,the patients with acute cerebral infarction were divided into four groups by observing the intimal thickness and the nature of the plaques:normal endometrium(Normal carotid artery group),endometrial thickening group,stable plaque group,unstable plaque group.The last three groups are carotid atherosclerosis.The plaque group were divided into mild carotid artery stenosis group and moderate and severe carotid stenosis group further.Use ELISA to detect the expression level of anti apoA-lIgG and MMP-9.Results The expression level of anti apoA-1 and MMP-9 in carotid atherosclerosis group was significantly higher than that in the normal carotid artery group(P<0.05).The expression level of anti apoA-lIgG and MMP-9 in the 4 groups increased successively,while PON-1 decreased in turn(P<0.01).There was a positive correlation between the serum level of anti apoA-lIgG and MMP-9 and the degree of carotid atherosclerosis(r=0.407,P<0.01;r=0.316,P<0.01).Taking IMT and plaque stability as dependent variable and atherosclerosis related risk factors as independent variables,logistic regression analysis,The results showed that anti apoA-1IgG and MMP-9 were independent risk factors for carotid atherosclerosis and plaque instability[(anti-apoA-1 IgG:OR=23.226,95%CI:4.204?128.305,P<0.01,MMP-9:OR=2.287,95%CI:0.890?5.872,P<0.05;anti apoA-1IgG:OR=3.301,95%CI:1.212?8.991,P<0.01;MMP-9:OR=2.345,95%CI:1.129-4.871,P<0.05)].PON-1 is the protective factor of carotid atherosclerosis(OR=0.814,95%CI:0.686?0.966,P<0.05);The expression level of anti apoA-lIgG and MMP-9 in carotid artery stenosis group was significantly higher than that in mild carotid artery stenosis group(P<0.01).The expression level of anti apoA-ligg was positively correlated with the degree of carotid artery stenosis(r=0.372,P<0.01).Moderate to severe carotid artery stenosis as dependent variable,the risk factors of with carotid stenosis as independent variables for logistic regression analysis,the results showed that anti apoA-1IgG is the independent risk factor for carotid artery stenosis(OR=5.296,95%CI:2.010?13.951,P<0.01).Correlation analysis showed a positive correlation between anti apoA-1 IgG and the expression level of MMP-9(r=0.523,P<0.01).The expression levels of anti apoA-1IgG and PON-1 was negatively correlated(r=-0.461,P<0.01).Conclusions Anti apoA-1 IgG and MMP-9 are independent risk factors for carotid atherosclerosis in patients with cerebral infarction,and are positively correlated with the degree of carotid atherosclerosis in patients with cerebral infarction.Moreover,anti apoA-1 IgG is an independent risk factor for severe carotid artery stenosis in patients with cerebral infarction,and is positively correlated with the degree of carotid stenosis in patients with cerebral infarction,PON-1 is the protective factor of carotid atherosclerosis.Detecting and regulating the concentration of the two may be a new way to detect early atherosclerosis.
