| ObjectiveLiterature research:a database was established based on the cloud platform of ancient and modern medical records,and various data mining methods were used to analyze the medication rule,drug attribute and core medication of prescription of ovulation disorders,providing new ideas for prescription medication of ovulation disorders and exploring the etiology and pathogenesis characteristics of the disease from prescription medication.Clinical study:observe the clinical efficacy of yufutang in treating ovulation disorder(anovulatory type),analyze the TCM syndromes of the included patients,explore the etiology and pathogenesis of the disease,summarize clinical experience,analyze its cubic basis,and provide certain clinical evidence for the clinical application of yufutang.MethodLiterature research:wanfang database,weipu database and cnki database were used to search and collect the prescriptions for ovulation disorders in literature on January 1,2015 solstice and January 1,2020.Input data,use the cloud platform of ancient and modern medical cases to analyze the medication data and explore the core drugs.Clinical study:a total of 42 patients with ovulatory dysfunction(anovulatory type)who were admitted to the international department of dongzhimen hospital of Beijing university of Chinese medicine between January 2019 and January 2020 were collected.Yuguang decoction was used as the basic prescription for 3 courses of treatment.During the treatment,the general situation of the patients and related indicators and scales were collected and analyzed.Results1.Literature research results:a total of 105 prescriptions were included,including 117 traditional Chinese medicines,20 of which were more than 20 times in frequency,most of which were plain and warm drugs,and mainly sweet,bitter and spicy.The drug mainly goes to the liver,kidney and spleen.>occurred 30 times in a total of 18 drug pairs.The five drug combinations with the highest frequency were:cuscuta-angelica,cuscuta-goji fruit,cuscuta-paeonia alba,angelica一goji fruit,paeonia alba-cooked rehmannia rehmannii.Three kinds of drugs were obtained through cluster analysis.The first one was dodder,angelica sinensis,radix paeoniae alba--nourishing kidney and blood.The second category:wine cornel meat,Chinese yam,wolfberry fruit,fructus ligustri-tonifying essence;The third category can be divided into 2 groups:the first group:shushan,parasitic mulberry,salvia miltiorrhiza,red peony root--tonifying kidney and activating blood;Group 2:others.Including waming kidney Yang,liver and spleen.Through parameter setting,get the core drugs:dodder,angelica,medlar,ripe ground,rhizome,radix paeoniae,radix paeoniae,wine corus,yam,ligusticum chuanxiong,rhizoma eucommiae,ligustrum,eucommia poria,Fried atractylodes,herba herba,salvia miltiorrhiza.2.Clinical research results:(1)Age distribution of patients:1 patient aged 20-25 years(3.23%),14 patients aged 26-30 years(38.71%),20 patients aged 31-35 years(45.16%),and 7 patients aged 36-40 years(12.90%).(2)A total of 17patients(40.48%)with a course of disease of less than 3 years,14 patients(33.33%)with a course of disease of 3-5 years,10 patients(23.81%)with a course of disease of 5-10 years,and 1 patient(2.38%)with a course of disease of more than 10 years.(3)Among the 42 included patients,35(83.33%)were PCOS patients,5(11.91%)were POI patients,1(2.38%)was hyperprolactinemia patients,and 1(2.38%)was hypothalamus and pituitary diseases patients.(4)The syndrome types of the included patients were analyzed,and all the 42 patients could be identified as kidney deficiency syndrome.According to the syndrome differentiation,there were 22 cases of liver stagnation,12 cases of blood stasis and 8 cases of phlegm and dampness.All the 42 patients had kidney deficiency.There were 8 cases of simple kidney deficiency,14 cases of kidney deficiency+liver stagnation,5 cases of kidney deficiency+phlegm dampness,7 cases of kidney deficiency+blood stasis,3 cases of kidney deficiency+liver stagnation+ phlegm dampness,5 cases of kidney deficiency+liver stagnation+blood stasis.(5)Eefficacy index:in this study,a total of 42 cases were included,10 cases were cured,22 cases showed obvious effect,9 cases were effective,1 case was ineffective,and the total effective rate was 97.62%.The TCM syndrome score before and after treatment was analyzed,and the TCM syndrome score after treatment was significantly lower than before treatment,with statistical significance(P<0.05).After the end of each course of treatment,according to the patient’s menstrual situation(period,period,menstrual volume)through the menstrual syndrome score table for evaluation,compared with the score after each course of treatment,there was a statistical difference(P<0.05).A total of 126 cycles were monitored.The monophasic body temperature was 48 cycles(38.10%).There were 31cycles(24.60%)of body temperature biphasic,but the duration was less than 12 days.There were 47 cycles(37.30%)of basal body temperature biphasic with a duration of 12-14 days.A total of 126 cycles were monitored,with ovulation occurring in 76 cycles(60.32%)and LUFS in 2 cycles(1.59%).Ovulation rate was compared between different courses of treatment,P<0.01 between course 1 and course 2,showing statistical difference.There was no significant difference between course 2 and course 3 at P>.05.There were 7 pregnancies,accounting for 16.67%of the total cases.Serum progesterone was measured in 36 patients with ovulation in the third course of treatment on day 5-7 of basal body temperature rise,with a mean value of 15.50±6.562ng/mL.Conclusion1.Literature research:the pathogenesis of ovulation disorder is complicated,and the treatment methods are mainly tonifying liver and kidney,tonifying kidney and nourishing blood,tonifying and nourishing decoction,supplemented by softening liver,tonifying spleen and stomach,activating blood and removing stasis.Commonly used drugs are nourishing liver and kidney,nourishing blood,soothing liver and so on.2.Clinical study:(1)The basic pathogenesis of this disease is kidney deficiency,accompanied by liver depression,blood stasis and phlegm and dampness.All kinds of syndromes co-appear and intermingle with each other.And when the treatment should follow the Yin and Yang transformation of each menstrual period,qi and blood ebb and flow characteristics of the drug.(2)Yuguang decoction has obvious effect on ovulation rate of patients.The menstrual cycle,menstrual period and menstrual volume were improved during treatment.The symptoms of TCM were significantly improved.And no obvious adverse reactions,can be further promoted in clinical application. |
