| Objective:To investigate the clinical effect of local drug injection combined with small needle scalpel in the treatment of acute radiculopathy of cervical spondylotic radiculopathy.Methods:Select 81 cases between August 2018 and November 2019 in the third affiliated hospital of Beijing medical university during orthopaedic clinic on the diagnosis of patients with cervical spondylosis of nerve root type,divided into injection group,the needle knife group and combined group,each group of 27 cases,respectively,using local drug injection therapy,small needle knife therapy and local drug injection in combination with small needle knife therapy intervention.The number of treatments was 1,and the evaluation period was 1 week.After treatment,the curative effect was recorded and statistically analyzed The scores of visual analogue scale(VAS)and cervical spondylosis clinical Evaluation Scale(CASCS)were recorded before and after treatment,and the clinical efficiency was recorded after treatment.In the course of treatment,we should pay close attention to the adverse reactions of the three therapies and timely deal with them,and summarize and evaluate the effectiveness of the three treatment methods.In this study,SPSS 20.0 was used for statistical analysis to determine the effectiveness of the treatment methods in the three groups.P<0.05 was considered statistically significant.Results:(1)there were no statistically significant differences in age(P=8.224>0.05),gender(P=0.801>0.05)and course of disease(P=1.031>0.05)among the three groups of CSR patients,indicating comparability(2)before treatment,CASCS scores of the three groups were compared,including clinical symptoms(P=0.684>0.05),subjective symptoms(P=0.559>0.05),adaptive ability(P=0.701>0.05)and total score(P=0.691>0.05).There was no statistical difference and the scores were comparable.Before treatment,VAS scores of the three groups were compared P=0.632>0.05,which showed no statistical difference and was comparable(3)After treatment,the total clinical response rate was 96.30%(26/27)in the combined group,significantly higher than that in the acupotomy group(66.67%)and injection group(70.37%),with statistical significance(P=0.018<0.05).The total effective rate of the injection group was higher than that of the acupotomy group,with statistical significance(P=0.015<0.05).(4)After treatment,intra-group comparison:CASCS scores of the three groups(specifically divided into 13 comparison items)were significantly improved compared with those before treatment and had statistical significance(All P values were less than 0.05).VAS scores of the three groups were significantly improved compared with those before treatment and were statistically significant(All P values were less than 0.05).The curative effect of the three groups was obvious.(5)after treatment,the comparison between groups:three groups of CASCS scores increase,combined score is better than that of needle knife group(including the clinical symptoms(P=0.001<0.05,the subjective symptoms(P=0.040<0.05,the ability to adapt P=0.016<0.05 and P=0.003<0.05 4),total score and is superior to the injection group(including the clinical symptoms(P=0.008<0.05,the subjective symptoms(P=0.021<0.05,the ability to adapt P=0.037<0.05,and total P=0.011<0.05 4);The scores of injection group were better than acupotomology group(including four items:clinical symptoms P=0.002<0.05,subjective symptoms P=0.048<0.05,adaptive ability P=0.018<0.05 and total score P=0.003<0.05).Namely,combination group>injection group>acupotomology group.As for the reduction of VAS scores in the three groups,the combined group was superior to the injection group(P=0.014<0.05)and the acupotomy group(P=0.010<0.05).The injection group was superior to acupotomology group(P=0.039<0.05).Namely,combination group>injection group>acupotomology group.Conclusion:(1)Local drug injection combined with small-needle scalpel therapy,local drug injection therapy and small-needle scalpel therapy can be applied in the treatment of CSR.After treatment,cervical spine mobility function can be improved(CASCS score can be increased)and pain can be reduced(VAS score can be reduced);(2)Local drug injection combined with small needle-knife therapy is superior to acupotomy and injection therapy in both alleviating pain(reducing VAS score)and improving cervical mobility(improving CASCS score);Namely,combination group>injection group,combination group>acupotomy group;(3)Local drug injection therapy alone is superior to acupotomology in alleviating patients’ pain(reducing VAS score)and improving patients’ cervical spine movement(improving CASCS score).Namely the injection group>acupotomology group. |
