Effect Of Modified Fuyuan Huoxue Decoction On Preoperative Swelling Of Rüedi-allg(?)wer Ⅱ、Ⅲ Type Of Tibial Pilon Fracture

Objective:To observe the effect of modified Fuyuan Huoxue Decoction on Pilon fracture Rüedi-Allg(?)wer II and III preoperative swelling treatment,and explore its clinical efficacy.Aiming to provide clinical basis for modified Fuyuan Huoxue Decoction in the treatment of Pilon fracture Rüedi-Allg(?)wer II and III preoperative swelling.To provide reference for the optimization of clinical treatment scheme.Methods: 60 cases of tibial Pilon fracture which were accepted by orthopedic department of Fuzhou Second Hospital were randomly divided into two groups.30 patients in the control group were treated with calcaneal traction,raised the affected limbs,took desmin and losoprofen,ice compress and exercise routine treatment;30 patients in the treatment group were treated by oral addition and reduction of Fuyuan Huoxue Decoction(the original prescription added Poria,Alisma,achyranthem,and pangolin).The changes of swelling rate of limbs,the time of occurrence of scab and dermatoglyphia,the dosage of painkillers,blood sedimentation and dynamic D-dimer were observed and recorded in 3,5 and 7 days after the treatment.The effects of Fuyuan Huoxue Decoction on the soft tissue swelling before operation of tibial Pilon fracture were analyzed.Results: A total of 60 patients were collected,including 19 males and 11 females in the treatment group,with an average age of 45 years old;There were 17 male patients in the control group.There were 13 female patients with an average age of 43 years old.There were no significant differences in gender,age and degree of soft tissue injury among the groups(P > 0.05).The degree of swelling elimination: the swelling rate(%)of the patients in the third,fifth and seventh days after the treatment was compared,P < 0.05,the difference was statistically significant,indicating that after three days of treatment,the results of the treatment group were significantly better than that of the control group in improving the swelling rate of the affected limbs.The time of appearance of dermatoglyphia and dry scab of blister: in terms of the time used in the appearance of dermatoglyphia and dry scab of blister,the days used in the treatment group were less than that of the control group,P < 0.05,indicating that the difference was statistically significant.The treatment group was significantly better than the control group in the appearance of skin pattern sign and dry scab time of blister.Analgesic dosage: when VAS pain score was less than 4.5 after the two groups were used,the use dose of painkillers was compared between the two groups,P < 0.05,the difference was statistically significant.It showed that the results of the treatment group were significantly better than that of the control group in improving the limb pain after the two groups were used.The blood red blood cell sedimentation rate: compared with the control group,the results of deep and shallow vein color ultrasound examination of the lower limbs were compared between the treatment group and the control group,which showed that there was significant difference(P < 0.05),indicating that the difference was statistically significant,and the treatment group was significantly better than the control group.Conclusion:On the basis of Fuyuan Huoxue Decoction,Poria,Alisma,Niugeng were added,and pangolin was subtracted to treat the tibial Pilon fracture Rüedi-Allg(?)wer II and III patients with preoperative soft tissue swelling,can effectively accelerate the elimination of limb swelling,promote the re epithelization of blisters or blood bubbles and the appearance of dermatoglyph signs,reduce pain,reduce the erythrocyte sedimentation rate,plasma D-dimer level and increase range,and have less side effects.
