| Objective: To analyze the related influencing factors of chronic constipation in the elderly with Yin deficiency and the characteristics of changes in intestinal microflora and fecal non-targeted metabolomics,in order to provide certain theoretical basis for the study of the pathogenesis of chronic constipation in the elderly with Yin deficiency and provide prevention and treatment strategies for clinical diagnosis and treatment.Methods: A total of 50 elderly patients with Yin deficiency syndrome with chronic constipation(CC-1 group)and 50 healthy controls(HC-1 group)who met the inclusion criteria and exclusion criteria were selected from the 940 th Hospital of the Joint Logist Support Force of the People’s Liberation Army(PLA)from May 2019 to March 2020.The subjects filled in the questionnaire mainly through observing their diet preference,exercise status,constipation main symptom score,Patient Assessment of Constipation Quality of Life scale(PAC-QOL),Somatic Self-rating Scale(SSS)and Pittsburgh Sleep Quality Index(PSQI)were used to analyze the related factors and characteristics of Yin deficiency syndrome of chronic constipation in the elderly.Fifteen patients were randomly selected from the CC-1group(CC-2 group)and six subjects from the HC-1 group(HC-2 group),and stool samples were collected from the CC-2 and HC-2.Full-length 16 S r RNA sequencing technology based on the Pac Bio platform was used to detect the fecal flora of the two groups.At the same time,non-targeted metabonomics method was used to detect the changes of fecal metabonomics and KEGG enrichment analysis of the subjects,and the changes of intestinal microflora and metabonomics in elderly patients with chronic constipation and Yin deficiency syndrome were compared and analyzed.Results:1.There was no statistical significance in age,gender and BMI between the healthy control group and the experimental group(P>0.05).2.Compared with the healthy control group,there were statistical differences between the elderly patients with chronic constipation Yin deficiency syndrome and the healthy control group in drinking less water and low dietary fiber(P<0.05),but there were no statistical differences in food intake and irregular diet(P>0.05).Compared with the healthy control group,there was statistical difference in the amount of exercise(P<0.05).The score of main symptoms of constipation,PAC-QOL score,SSS scale score and PSQI score were significantly higher than those of healthy control group(P<0.05).3.Compared with the healthy control group,the abundance of Bacteroidetes and Fusobacterium in the intestinal microflora of the elderly patients with chronic constipation syndrome of Yin deficiency was lower than that of the healthy control group(P<0.05),while the abundance of Firmicutes and Microflora were higher than that of the healthy control group(P<0.05).There was no significant difference in the abundance of Proteobacteria and Actinobacteria compared with the healthy control group(P>0.05).4.Compared with the healthy control group,there were 12 different metabolites in the elderly patients with chronic constipation syndrome of Yin deficiency(P<0.05),and there were differences in the concentration of cysteine methionine metabolism,biosynthesis of secondary metabolites,biodegradation and metabolism of three metabolic pathways between the two groups.Conclusion:1.The main factors affecting chronic constipation in the elderly with Yin deficiency include less drinking water,low dietary fiber,less exercise,poor mental and emotional state,poor sleep quality and so on.2.Compared with the healthy control group,the elderly patients with chronic constipation with Yin deficiency had intestinal microflora imbalance,and their intestinal microflora had a trend change.3.Fecal metabonomics of elderly patients with chronic constipation of Yin deficiency had a tendency to change compared with that of healthy controls. |
