Study On The Medication Regularity Of Blood Activating Herbs In Diabetic Nephropathy Based On Data Mining

Objective:In this project,the traditional Chinese Medicine Inheritance assistant system is applied to collect and sort out the literature on traditional Chinese medicine treatment of diabetic kidney disease(DKD)from CNKI after 2016,and establish the original database of DKD prescriptions.On this basis,the blood activating traditional Chinese medicine is extracted,and the original data and the medication characteristics and rules of blood activating traditional Chinese medicine are analyzed from various angles and aspects,so as to provide reference for the treatment of diabetic kidney disease To provide some guidance or reference for the application of diseases and blood activating herbs.Methods:In this study,"diabetes nephropathy,diabetic nephropathy,traditional Chinese medicine" and other keywords were used to collect and collate the literature on traditional Chinese medicine treatment of diabetic nephropathy from CNKI after 2016.The medication information was entered into the traditional Chinese Medicine Inheritance assistant system(v2.5).The frequency analysis,association rules,cluster analysis methods were used to analyze the database of traditional Chinese medicine,and the results were analyzed According to the efficacy of traditional Chinese medicine in Chinese Pharmacopoeia and traditional Chinese medicine,the blood activating drugs in DKD were screened by spss21.0 Software,using data mining technology to classify,frequency analysis,high-frequency dosage and staging(stage Ⅲ,Ⅳ,Ⅴ)cluster analysis of blood activating drugs,to find the medication rule of blood activating drugs in diabetic kidney disease.Result:1.The data of diabetic kidney disease(DKD)were collected from China National Knowledge Infrastructure(CNKI),and 563 literatures met the inclusion criteria,including 431 definite stages and 46 medical records,involving 298 traditional Chinese medicines.The top 10 Chinese medicines were Astragalus membranaceus,Poria cocos,Salvia miltiorrhiza,Chinese yam,Cornus officinalis,Rehmannia glutinosa,Alisma orientalis,Angelica sinensis,Atractylodes macrocephala and Rehmannia glutinosa;the top 10 medicines were tonifying deficiency herbs,promoting blood circulation and removing stasis herbs,clearing heat herbs,diuresis and dampness herbs,astringent drugs,relieving exterior syndrome herbs,Purgative Herbs,calming liver and calming wind herbs,dispelling wind and dampness herbs and warming interior herbs;the frequency of four Qi was arranged The frequency of five flavors:sweet,bitter,pungent,sour Astringent,salty;channel into the liver,spleen,kidney is the most,followed by the heart,lung,five viscera,six Fu organs for the second;through the"formula analysis" function,after systematic calculation,33 pairs of drug combinations with frequency more than 120 are obtained,and the association rules involve 13 core drugs:Astragalus,Poria cocos,yam,Salvia miltiorrhiza,Cornus officinalis,rehmannia,Alisma orientalis,chuanxiong,rehmannia,angelica,rhubarb Based on entropy clustering of complex system,32 core combinations were evolved from the database,and 16 potential new prescriptions were obtained by further cluster analysis.Through further analysis,four more significant new prescriptions were obtained:① tortoise shell,donkey hide gelatin,antler glue and Begonia seed;②GUI,astragalus,Rosa laevigata,papaya and gypsum;③peach kernel,Bombyx Batryticatus,safflower,dragon’s blood and golden buckwheat;④Smilax glabra,pangolin,Magnolia officinalis,golden buckwheat,Imperata root and caokuo.2.The traditional Chinese medicines for promoting blood circulation in DKD database were classified as:drugs for promoting blood circulation and regulating menstruation,drugs for relieving pain,drugs for treating injury,drugs for breaking blood and eliminating symptoms,drugs for promoting blood circulation and other drugs,drugs for dredging collaterals;the top 10 drugs for promoting blood circulation in the database were Salvia miltiorrhiza,Angelica sinensis,rhubarb,Ligusticum chuanxiong,Cortex Moutan,leech,earthworm,Pueraria,safflower and Leonurus;the prescriptions containing Salvia miltiorrhiza had specific dosage(hereinafter referred to)The treatment window was 9-50g,the average dose was(18.65±7.32)g,p10-p90 was 10-30g,and the most commonly used dose was 15g.The proportion of prescriptions exceeding the upper limit of Pharmacopoeia accounted for 42.86%.The average dosage was(14.13±5.25)g,p10-p90 was 10-20g,and the most commonly used dosage was 15g.The over dosage rate was 50.68%.There were 121 prescriptions containing rhubarb,the treatment window was 3-30g,the average dose was(8.92±3.87)g,p10-p90 was 5-15g,the most commonly used dose was 10g,and the over dose rate was 48.76%.There were 117 prescriptions containing Chuanxiong.The treatment window was 6-90g.The average dose was(14.30±10.5)g.p10-p90 was 10-18g.The most commonly used dose was 15g.The over dose rate was 64.10%.There were 91 prescriptions containing Cortex Moutan.The treatment window was 5-18g,the average dose was(11.90±2.64)g,p10-p90 was 9-15g,the most commonly used dose was 10g,and the over dose rate was 37.36%.There are 86 prescriptions containing leech,the treatment window is 1-30g,the average dose is(5.78±4.32)g,p10-p90 is 2.7-1 0g,the most commonly used dose is 3G,and the over prescription rate is 53.49%.In DKD stage,there are 289 prescriptions of blood activating herbs in stage Ⅲ,with a total of 51 ingredients.Through cluster analysis,eight core groups are obtained,which constitute four new prescriptions.There are 164 prescriptions in stage Ⅳ,with a total of 50 flavors.Through cluster analysis,10 core combinations are obtained,which constitute 5 new prescriptions.There are 23 prescriptions and 28 flavors in stage v.After cluster analysis,six core combinations were obtained,which formed three new squares.Conclusion:1.The application of DKD is related to liver,spleen and kidney;2.Drugs mainly focus on sweet,warm and deficiency tonifying drugs(mainly Qi tonifying and blood tonifying drugs),bitter and cold blood activating and stasis removing drugs and heat clearing drugs;3.Commonly used drugs for DKD:Astragalus membranaceus,Poria cocos,yam,Salvia miltiorrhiza,Cornus officinalis,Rehmannia glutinosa,Alisma orientalis,Ligusticum chuanxiong,Rehmannia glutinosa,Angelica sinensis,rhubarb,Atractylodes macrocephala and Codonopsis pilosula;4.Four significant new prescriptions were obtained in the treatment of DKD,which were mainly nourishing liver and kidney,strengthening spleen and stomach,strengthening kidney,clearing turbidity and Removing Stasis Toxin;5.In DKD,drugs for activating blood circulation can be divided into three categories:drugs for activating blood circulation to remove blood stasis,drugs for activating blood circulation to other categories,and drugs for dredging collaterals;drugs for activating blood circulation to other categories mainly focus on clearing away heat,while drugs for dredging collaterals mainly focus on calming wind and removing dampness;6.The dosage of blood activating drugs is constant,variable and excessive.The clinical prescription selection should be flexible and changeable;7.For the new prescription of promoting blood circulation,the prescription can be selected according to the TCM classification of DKD and the complications of Western medicine,but its effectiveness needs further clinical verification.
