Objective:To explore the value of platelet to lymphocyte ratio(PLR),D-dimer and CA125 in the diagnosis of ovarian cancer and their relationship with clinicopathological characteristics.Methods:A total of 102 patients who underwent gynecological surgery at the hospital with pathological diagnosis of epithelial ovarian cancer from January 2007 to November 2019 were selected as the ovarian cancer group.107 patients with pathological diagnosis of benign tumor were as benign tumor group,while 110 healthy female were selected as control groups.Information of patients like general information,blood routine,D-dimer,carbohydrate antigen 125(CA125)serum levels as well as tumor histology type,clinical stage and some other information were selected in the ovarian cancer group and benign tumor group;General information and blood routine of the control group were selected too.Kruskal-Wallis nonparametric test method was used to test the significance of differences between groups,and Bonferroni correction method was used to perform multiple comparisons between groups.The Mann-WhitneyU nonparametric test method was used for comparison between the two groups.Univariate analysis and multivariate logistic regression analysis were used to find indicators related to the diagnosis of ovarian cancer.The receiver operating characteristic(ROC)curve and the area under the ROC curve(AUC)were used to evaluate the value of PLR,D-dimer and CA125 in the diagnosis of ovarian cancer.Results:(1)The platelet and PLR of ovarian cancer group were higher than that of benign tumor group and healthy control group,and the difference was statistically significant(P<0.05);The lymphocyte count of ovarian cancer group was lower than that of benign tumor group and healthy control group,and the difference was statistically significant(P<0.05).The CA125 and D-dimer values of ovarian cancer group were higher than those of benign tumor group,and the difference was statistically significant(P<0.05).(2)Through Logistic regression curve analysis,PLR and D-dimer are relevant factors for the diagnosis of ovarian cancer.(3)After ROC curve analysis,the areas under the curve of PLR,D-dimer and CA125 were 0.707,0.895 and 0.908,respectively,all with statistical significance(P<0.01);With PLR sensitivity of 41.2%,specificity of 86.9%;D-dimer sensitivity of 52.0%and specificity of 99.1%;And with CA125 sensitivity of 77.5%and specificity of 90.7%;The best cutoff values are 160.563,1.195,and 39.150,respectively.(4)Through ROC curve analysis,the area under the curve of CA125+D-dimer,CA125+PLR,PLR+D-dimer,CA125+D-dimer+PLR is 0.913,0.910,0.9050.916 respectively.The sensitivity is 78.4%,71.6%,78.4%,79.4%respectively,and the specificity is 97.2%,96.3%,94.4%,97.2%respectively.(5)D-dimer value of ovarian cancer stage Ⅲ+Ⅳ group was significantly higher than that of stage Ⅰ+Ⅱ group,the difference was statistically significant(P<0.05);CA125 in ovarian cancer stage Ⅲ+Ⅳ group was higher than that in stage I+II group,and the difference was statistically significant(P<0.05).There was no significant difference in PLR value between ovarian cancer stage Ⅲ+Ⅳ group and stage Ⅰ+Ⅱ group(P>0.05);D-dimer in ovarian cancer lymphatic metastasis group was higher than that in no lymph node metastasis group with statistically significance(P<0.05);CA125 in the ovarian cancer lymph node metastasis group was higher than that in the non-lymph node metastasis group,with statistically significant difference(P<0.05).The histological type of CA125 in serous carcinoma group was higher than that in non-serous carcinoma group,with statistically significant difference(P<0.05).There was no statistically significant difference in PLR value between the presence or absence of lymph node metastasis(P>0.05).There was no significant difference in D-dimer,CA125 and PLR showed no statistically significant differences among different age groups and ethnic groups(P>0.05).There was no statistically significant difference between d-dimer and PLR in histological type of ovarian cancer(P>0.05).Conclusions:(1)PLR,D-dimer and CA125 can be used as ovarian cancer screening indicators;the combined application of the three indicators can improve the diagnosis rate of ovarian cancer.(2)D-dimer and CA125 can be used as the basis for clinical staging of ovarian cancer.
