| Object:To understand the current situation of the medical treatment system construction of public health emergencies in Henan Province.To comprehensively evaluate the ability of hospitals of secondary and above in Henan Province to respond to public health emergencies.To discuss the problems existing in emergency capacity construction and analyze the advantages and disadvantages,so as to provide a basis for hospitals,health administrative departments and the government to formulate relevant policies and provide reference for the improvement of the ability of hospitals to respond to public health emergencies in Henan Province.Methods:The questionnaire survey was used in this study to survey the current situation of public health emergency response ability of secondary and above hospitals in Henan Province.On this basis,the horizontal evaluation combined with the literature research was used to determine the evaluation indexes.The entropy method was used to determine weights,and weighted TOPSIS method,weighted rank-sun ratio and the factor analysis method were used for comprehensive evaluation.The ten-year comparison was used as a longitudinal evaluation.Results:1.Among the secondary and above hospitals,the number of people with master’s degree or above accounted for 7.3% of the total.And the number of people with intermediate or above professional title accounted for 37.25% of the total.The proportion of public health medical treatment subsidies received by hospitals in the financial subsidies from the government and health administrative departments is higher in tertiary general hospitals than in secondary general hospitals.A total of39.25% hospitals raised emergency funds for public health emergencies in the past five years,with an average of 95.1455 million yuan,and a total of 27.17% hospitals could meet the needs of emergency funds.A total of 88.87% hospitals had established information management systems.A total of 93.58% hospitals had leaders in charge of public health treatment,and a total of 92.45% hospitals had formulated emergency plans for public health treatment.In the past three years,a total of 84.72% hospitals’ staff received training on public health incidents,and a total of 76.98% hospitals carried out or participated in public health medical treatment drills.When the number of patients seeking medical treatment for a certain disease or symptom exceeded the usual level,a total of 94.91% hospitals required doctors on duty to report to the general duty or specific person in charge of the hospital.A total of 70.75% hospitals were designated as hospitals/stations for urgent care centre,and a total of 62.64%hospitals had experts in public health treatment expert groups at all levels.2.The ranking results of weighted TOPSIS method,weighted rank-sum ratio method and factor analysis method were statistically significant(P < 0.05).The comprehensive evaluation results showed that the top-ranked hospital was Zhengzhou tertiary general hospital with a comprehensive score of 1.8122,and the last-ranked hospital was San Menxia secondary specialized hospital with a comprehensive score of 0.3367.The average comprehensive score of the 530 hospitals was 0.7935.Zhumadian(0.8773)had the highest average comprehensive score,Sanmenxia(0.7190)had the lowest.The average comprehensive scores of hospitals of different grades were 0.9997 for tertiary hospitals and 0.7552 for secondary hospitals.The average comprehensive scores of different types of hospitals were 0.8463 for general hospitals and 0.6714 for specialized hospitals.3.Compared to 2010,the proportion of physicians and registered nurses increased significantly in 2019.The proportion of government and health administrative departments’ financial subsidies used for public health medical treatment had decreased.In the public health medical treatment expenditure,the proportion of medical and office staff subsidies and logistics services,transportation and other expenditure increased,while the proportion of medical and drug expenditure decreased.The number of hospitals raising emergency funds by themselves increased significantly.The construction rates of hospital information management system,telemedicine system and emergency coordination,emergency materials reserve and emergency medical first-line subsidy system increased.The number of full-time and part-time staffs in hospitals were responsible for reporting infectious diseases and public health incidents increased.The detection rates of Hepatitis E virus,Enterovirus 71,Influenza A(H1N1)virus,Vibrio cholerae,Shigella dysenteriae,typhoid and paratyphoid bacilli,Escherichia coli,Neisseria meningitidis,Brucella and organophosphorus poisoning were increased.The number of hospitals setting up emergency treatment plans for emerging infectious diseases or group diseases with unknown causes increased.The number of hospitals setting up expert groups and emergency treatment plans for outbreaks of foodborne diseases,outbreaks of iatrogenic infections and abnormal reactions to mass vaccination increased.Conclusions:In 2019,the ability of secondary and above hospitals in Henan to respond to public health emergencies had been significantly improved,but there were still some shortcomings.The professional construction of talent team was not strong.The emergency training and drill was not enough.The funds for public health medical treatment and emergency supplies were insufficient.The level of information exchange and sharing was low.The ability of laboratory detection was weak.Therefore,it is necessary to further strengthen the construction of talent team,perfect the fund guarantee mechanism,perfect the emergency material reserve,strengthen the construction of information,and improve the capacity of public health detection and treatment. |
