Research purposes:Conduct a investigation about cancer pain awareness of doctors in three hospitals in Henan Province,and trained by the same experts in accordance with the quality control standards for cancer pain in Henan Province,check the cancer pain cases before the training and at two different time periods after the training,evaluate the impact of cancer pain quality control supervision on the effectiveness of standardized treatment of cancer pain in three hospitals.research method:1.Develop a questionnaire reviewed by experts on the awareness of cancer pain,understand the cancer pain awareness of relevant medical staff through questionnaire survey method.2.Provide training about cancer pain knowledge to relevant doctors by means of on-site lectures.3.Investigate the cancer pain treatment status in three time periods through themethod of on-site evaluation of electronic cases,including seven observation indicators including cancer pain diagnosis,analgesic drug selection,drug titration,pain dynamic assessment,burst pain management,NSAIDS drug risk assessment,and pain medication side effects management.4.Collect data and conduct statistical analysis by SPSS 20.0 software.All measurement data obey normal distribution and expressed by mean± standard deviation;The method about Count data is chi-square test of multiple sample rates,p<0.05 is considered statistically significant;In addition,the Bonferroni method was used for pairwise comparison between multiple groups.Research result:1.The survey results of doctors’treatment behavior and cognition about cancer pain:There are 33 doctors participated in the survey,including 12 male doctors and 21 female doctors.84.8%of doctors believe that the training and teaching of cancer pain knowledge has a great influence on the improvement of diagnosis and treatment behavior;In terms of pain screening:The commonly used pain assessment methods are the digital assessment scale(63.6%),the facial expression assessment scale(27.3%),and the speech description scale(9.1%);A small number of people(27.3%)cannot accurately use the neuropathic pain screening form.In terms of the incidence of cancer pain:There are 33.3%of people underestimated the true incidence of severe cancer pain.Regarding the use of opioids:A small number of people(15.2%)cannot accurately remember the conditions of opioid tolerance;The vast majority of people(84.8%)can better understand the dose conversion between different types of opioids.For the analysis of the primary cause of poor pain control:Insufficient dosage of analgesics(51.5%),too strict management of analgesics(18.2%),failure to take medications on time(15.2%),high costs(9.1%),and others(6.1%).2.Comparing the treatment of cancer pain in three time periods,the results are:2.1 Overall comparison:The following 7 observation indicators all have statistical differences:Cancer pain diagnosis(χ2=14.426,P=0.001),analgesic drug selection(χ 2=19.076,P=0.000),analgesic titration(χ2=7.4695,P=0.024),pain dynamic assessment(χ2=7.530,P=0.023),outbreak pain management(χ2=8.051,P=0.018),risk assessment of using NSAIDS drugs(χ2=26.329,P=0.000),treatment of side effects of analgesic drugs(χ2=44.688,P=0.000).2.2 Pairwise comparison between the three time periods:2.2.1 For cancer pain diagnosis,analgesic titration,burst pain management and pain dynamic evaluation:The comparison between the first time period and the third time period is statistically significant;However,there are no statistical significance between the first time period and the second time period,between the second time period and the third time period.2.2.2 For analgesic choice and the side effects of analgesics:There are statistical significance between the first time period and the third time period,between the second time period and the third time period;But there is no statistical significance between the first time period and the second time period.2.2.3 For the risk assessment of NSAIDS drugs:The comparison between the first time period and the second time period,between the first time period and the third time period are statistically significant;But there is no statistical significance between the second time period and the third time period.Analysis conclusion:1.The Cancer Pain Quality Control and Supervision Standards of Henan have a positive impact on the standardized cancer pain diagnosis and treatment behaviors of relevant medical staff in the three hospitals,and have reference significance for the further promotion of the standardized cancer pain treatment.2.Regarding the evaluation indicators of cancer pain treatment status:none of the pass rates have reached the standards required by the Henan Cancer Pain Control Center,and there is still a need to increase the intensity of standardized management of cancer pain.
